Dosing and titration
Hypertension or angina pectoris
- Usual initial dose: 5 mg once daily
- If necessary, increase to 10 mg once daily
- Maximum recommended dose: 20 mg once daily
Stable chronic heart failure
- Start: 1.25 mg once daily
- Stepwise increase to 2.5 mg, 3.75 mg, 5 mg, 7.5 mg, 10 mg once daily
- Intervals: two weeks or longer, as tolerated
Renal or hepatic impairment
- In severe renal impairment (creatinine clearance less than 20 ml per minute) or severe hepatic impairment, daily dose of 10 mg must not be exceeded
