A recent observational study highlights that children born with very low birth weight face a significant burden of cognitive impairments, behavioral difficulties, and social adjustment challenges during their school-age years, specifically demonstrating markedly lower full-scale intelligence scores and elevated anxiety compared to peers born at normal weights, as published in the journal Indian Pediatrics in August 2025.

While recent strides in neonatal intensive care have markedly improved the survival rates of extremely fragile infants, a critical clinical gap remains regarding the long-term neurodevelopmental and psychological health of these survivors; therefore, Kanya Mukhopadhyay and her colleagues from the Department of Pediatrics conducted the research to evaluate how these early life challenges manifest as cognitive and behavioral outcomes during middle childhood.

Therefore, the observational study utilized a cohort design to assess the first 50 very low birth weight (VLBW) children—defined as those weighing 1250 grams or less—from an original 2009–2011 birth group who consented to follow-up, comparing them against 50 age- and sex-matched normal birth weight (NBW) controls between the ages of 6 and 11 years. To ensure a comprehensive clinical picture, a certified psychologist evaluated primary and secondary endpoints, including intelligence quotients, memory parameters, psychopathology, self-concept, and adjustment, using standardized tools such as Malin's Intelligence Scale for Indian Children (MISIC), the Childhood Psychopathology Measurement Schedule, and the PGI Memory Scale for Children (PGIMC).

Key Clinical Findings of the Study Includes:

  • Significant Cognitive Deficits: The study found that very low birth weight survivors achieved significantly lower scores in Verbal Intelligence Quotient (P = 0.008), Performance IQ (P < 0.001), and Full-Scale IQ (P < 0.001) when compared to their normal birth weight counterparts.

  • Impaired Memory Function: Participants in the VLBW cohort demonstrated markedly reduced performance across several domains, including remote memory (P < 0.001), mental balance (P = 0.001), attention (P = 0.005), and visual retention (P < 0.001), according to the study data.

  • Increased Behavioral Vulnerability: Higher clinical scores for psychopathology (P = 0.036) and anxiety (P = 0.034) were observed in the VLBW group, suggesting a heightened risk for emotional and psychological distress in this population.

  • Poor Social and Self-Adjustment: The research revealed that VLBW children had significantly lower self-concept and general adjustment scores (P < 0.001), indicating substantial hurdles in developing a positive self-image and adapting to social environments.

The results suggest that very low birth weight is a potent predictor of persistent neurodevelopmental vulnerabilities, with data indicating that these children struggle with a broad spectrum of cognitive and behavioral deficits that do not resolve by middle childhood.

These findings suggest that clinicians should prioritize the implementation of rigorous, long-term developmental surveillance and integrated psychological support for VLBW survivors to address these multifaceted challenges during their school years.

While the study provides valuable longitudinal insights into neonatal outcomes, the relatively small sample size underscores the need for larger, multi-center prospective studies to more definitively characterize the evolving neurodevelopmental needs of this high-risk group.
