Local Statin Therapy Shows Promise in Improving Periodontal Outcomes: Study
A new meta-analysis published in the American Journal of Dentistry found that locally delivered simvastatin and rosuvastatin used along with scaling and root planing significantly improved periodontal health, reducing pocket depth and enhancing clinical attachment level, gingival index, modified sulcus bleeding index, and relative attachment level. However, further well-designed clinical trials are needed to confirm these findings and strengthen the evidence base.
Chronic periodontitis affects the tissues that support teeth and remains a leading cause of tooth loss among adults worldwide. Standard treatment typically involves scaling and root planing (SRP), a deep-cleaning procedure aimed at removing plaque and calculus from tooth surfaces and periodontal pockets. However, studies continue to explore adjunctive therapies that can enhance healing and tissue regeneration.
The latest analysis reviewed evidence published through June 2025, and evaluated the effectiveness of locally administered simvastatin and rosuvastatin in periodontal treatment. An extensive search of PubMed, Scopus, Google Scholar, and ClinicalTrials.gov, helped in identifying randomized controlled trials that met strict inclusion criteria. After screening titles, abstracts, and full-text articles, a total of 30 randomized controlled trials involving 1,211 participants were included in the final analysis.
The patients receiving simvastatin or rosuvastatin in combination with scaling and root planing experienced notable reductions in periodontal pocket depth. Improvements were also observed in clinical attachment level, which suggested enhanced reattachment of periodontal tissues to tooth surfaces.
Additional benefits included significant reductions in gingival inflammation, as measured by the gingival index, and decreased bleeding upon probing, reflected by improvements in the modified sulcus bleeding index. Also, gains in relative attachment level were reported, further supporting the regenerative potential of these locally delivered medications.
Rosuvastatin demonstrated particularly strong effects across several clinical parameters, while simvastatin also consistently outperformed control treatments. However, the analysis identified that rosuvastatin did not show a statistically significant advantage in reducing plaque index scores, indicating that plaque control remains largely dependent on oral hygiene practices and mechanical cleaning methods rather than pharmacological intervention alone.
Previous studies highlight the anti-inflammatory, bone-stimulating, and tissue-regenerative effects of statins, characteristics that may contribute to improved periodontal healing when applied directly at disease sites. Overall, the study suggests that locally delivered simvastatin and rosuvastatin could emerge as valuable adjunctive therapies in periodontal care.
