A new study published in the journal of Diabetes Care showed that a diet similar to DASH but adjusted for diabetes led to a notable decrease in fructosamine, fasting glucose, and HbA1c levels in adults with type 2 diabetes when compared to a standard American diet.

Type 2 diabetes (T2D) presents a significant worldwide health issue marked by prolonged high blood sugar levels and worsening metabolic irregularities, heightening the chances of heart, kidney, and other issues. Altering one's diet is crucial for managing diabetes, with a focus shifting towards dietary patterns rather than specific nutrients. The Dietary Approaches to Stop Hypertension (DASH) diet, abundant in fruits, vegetables, whole grains, legumes, and low-fat dairy items, has proven advantages for managing blood pressure and overall heart health. Nevertheless, its impact on controlling blood sugar levels in people with T2D is not as clearly defined.

The DASH4D diet is a variation of the DASH eating plan designed for adults with type 2 diabetes. It involves reducing carbohydrates, increasing protein and unsaturated fats, and focusing on nutrient-rich foods. These adjustments could enhance insulin sensitivity and glucose control while addressing heart disease risk factors. Markers like glycated hemoglobin (HbA1c), fasting blood sugar, and post-meal sugar levels are important for monitoring metabolic health. Therefore, studying how the DASH4D diet affects these markers can help determine its effectiveness in managing blood sugar in adults with T2D.

During this monitored feeding study, individuals who had type 2 diabetes were given four different diets in a random sequence: DASH4D diet and a comparison diet (reflective of a standard American diet), both with varying levels of sodium. Each diet phase lasted for 5 weeks. By applying a modified intention-to-treat method, the impact of the DASH4D diet (compared to the comparison diet) on fructosamine, fasting glucose, and HbA1c was assessed using linear mixed-effects models.

In a group of 101 individuals (average age 67 years, 65% female, 87% African American adults), the DASH4D diet showed a notable decrease in end-of-period fructosamine (adjusted difference: −5.6 μmol/L, P = 0.002), fasting glucose (adjusted difference: −4.5 mg/dL; P = 0.02), and HbA1c (adjusted difference: −0.09 percentage points; P = 0.04) compared to the control diet.

The most significant decreases in fructosamine and fasting glucose levels were seen in individuals who had initial HbA1c levels of 7% or above. In general, this research backs the suggestion to use the DASH4D diet for controlling blood sugar in type 2 diabetes.
