Glycemic Variability Marker Linked To Higher Neovascular Glaucoma Risk In Proliferative Diabetic Retinopathy Patients: Study

The aim of the study was to explore the relationship between SHR and the development of NVG among patients with PDR and assess whether certain demographic and clinical characteristics modified this risk. 1,245 patients with PDR were included in a retrospective cohort study using electronic health records data from one hospital over a 10-year period from 2010 to 2020.
For the estimation of SHR, researchers used admission glucose in relation to the estimated average glucose from the HbA1c value. Logistic regression and Cox proportional hazards models were used to establish the intensity of association between SHR and NVG, controlling for the possible confounding variables.
Key Findings
•Of the 1,245 patients studied, 378 subjects (30.3%) were diagnosed with NVG. The mean SHR in the group was 2.9 overall, but NVG patients had a significantly higher mean SHR (3.2) than non-NVG patients (2.7) with a p<0.001.
• Multivariate logistic regression showed a significant correlation between SHR and NVG, with an odds ratio (OR) of 2.5 (95% CI, 1.9–3.3; p < 0.001), which means that increased SHR doubled the chance of developing NVG in PDR patients.
• Among males, the hazard ratio (HR) for NVG against SHR was 1.4 (95% CI, 1.1–1.7; p = 0.01).
• Among individuals above 65 years of age, the HR was 1.5 (95% CI, 1.2–1.9; p = 0.001).
• For patients with diabetes duration more than 15 years, HR was 1.4 (95% CI, 1.1–1.8; p = 0.01).
These results indicate that the effect of glycemic stress is more significant in older people, long-duration diabetics, and male patients, indicating the necessity of subgroup-specific monitoring.
The study authors conclude a definite and strong correlation between stress hyperglycemia ratio and neovascular glaucoma risk in proliferative diabetic retinopathy patients. These results indicate that SHR can be a useful clinical biomarker for early detection of patients at increased risk of NVG to guide more active and individualized prevention and management strategies.
