CPS Releases Declaration Form For Students Receiving Degree, diploma In Absentia
Mumbai: Through a recent notice, the College of Physicians and Surgeons of Mumbai (CPSM) has asked all the concerned students who were admitted In Absentia as a holder of the Fellowship/ Diploma/ Certificate at the College Convocation held on Friday, 08th October 2021 to fill the declaration form.
As per the notice, It is informed to all the concerned students that the Declaration Form (attached below) consists that "Under By-Law No. 3 of Section XXIII of the College By-Laws, every holder has to make and sign a declaration before receiving the Fellowship / Diploma/ Certificate. This declaration has to be signed in the College Roll. As you were admitted in absentia, the office is forwarding herewith a sheet of declaration applicable to you and a slip with a request that should be signed by you and returned to this office.
On receipt of the slip, signed as stated above, the slip will be filed in the College Roll and your Fellowship / Diploma/ Certificate will be forwarded to the students by post/courier.
Kindly send Rs.300/- (Rupees Three Hundred only) by Demand Draft in favour of "College of Physicians and Surgeons of Mumbai" payable at Mumbai of being the postal charges to send the certificate by courier/post. Along with a copy of self- attested photo & address identity proof once (Passport / Aadhar Card / Pan Card / Election Card / MMC Id card).
It is informed to all the eligible students that the last date to send the declaration to the CPS office, Mumbai is 30-Oct- 2021.
