CPS Mumbai Releases Declaration Form For medicos receiving Degree, Diploma In Absentia
Mumbai: College of Physicians & Surgeons (CPS) released the declaration form for students receiving their Degree/Diploma in Absentia.
The convocation ceremony was held on 8th July 2022, and many students have applied for Absentia mode to receive their Degree/Diploma Certificate. These students must make and sign a declaration before receiving the Fellowship/Diploma/Certificate under By-Law Section No.3 of Chapter XV of the College By-Laws.
This declaration has to be signed in the College Roll. CPS has released the sheet of declaration applicable to the students and a slip with a request that they should sign and return to CPS. On receipt of the slip, signed, the slip will be filed in the College Roll, and the Fellowship/Diploma/Certificate will be forwarded to the students by post/courier.
The students need to send a DD of Rs 300 in favour of the "College of Physicians and Surgeons of Mumbai" payable at Mumbai of being the postal charges to send the certificate by courier/post. They also need to send a copy of a self-attested government-approved photo & address identity proof once (Passport/Aadhar Card/Pan Card/Election Card) & MMC Id card.
Further, CPS has announced that 31st July 2022 is the last date to send the declaration. The mobile number and address with pin code should be clearly mentioned where the students want to receive their Fellowship / Diploma/ Certificate. The detailed declaration form is enclosed in the notice below.
College of Physicians & Surgeons of Mumbai, established in 1913, is an autonomous body imparting Postgraduate Medical Education and offers 10 Fellowships, 17 Diplomas, and 2 Certificate Courses. The qualifications granted by CPS Mumbai allow the Practitioner to register themselves as specialists in the specialty concerned upon completing the tenure of 2 years Diploma or 3 years Fellowship and passing the prescribed exit examination.
