Rethinking Recovery After Cesarean Section

With childbirth experiences under the microscope due to declining fertility rates, a fresh study published in BMC Anesthesiology delves into whether "Enhanced Recovery After Cesarean Section" (ERAC) protocols truly make a difference for mothers. While ERAC is already a game-changer in many surgical fields, its impact on postpartum recovery and patient experience after C-section has been less explored—until now.

What Was the Study About?

Researchers at Guangzhou Women and Children’s Medical Center conducted a randomized controlled trial with 140 women scheduled for elective cesarean deliveries. Participants were split into two groups: one following the ERAC protocol, and the other receiving standard care (SOC). ERAC included strategies like:

Reduced fasting times

Early mobilization and functional exercises

Earlier removal of urinary catheters

Multi-modal pain relief

Proactive nausea prevention

The main outcomes measured were hospital length of stay (LOS) and the Obstetric Quality-of-Recovery score (ObsQoR-11), a patient-centered indicator of recovery quality.

Surprising Results: Experience Up, Not Out the Door Sooner

Despite ERAC’s promise, the actual length of hospital stay was virtually identical in both groups—around 55 hours. However, the ObsQoR-11 scores told a different story: women in the ERAC group reported significantly better recovery experiences, with a median ObsQoR-11 score 17 points higher than those receiving standard care. Mothers benefitting from ERAC also experienced:

Faster return to movement after surgery

Earlier return of bowel function

Lower pain levels at 24 and 36 hours post-op

Importantly, these improvements in well-being didn’t come with any uptick in complications, costs, or adverse effects.

Traditional metrics like hospital stay and direct costs may not fully capture the postpartum experience. This study underlines the value of patient-reported outcomes—such as pain, mobility, comfort, and emotional well-being—which matter deeply to new mothers. By embracing ERAC, hospitals can prioritize what counts most for women: how they feel, recover, and bond with their babies.
