USA: A research letter published in JAMA Pediatrics provides new evidence supporting the effectiveness of annual influenza vaccination in children. The study found that young children born earlier in the year were more likely to receive the flu vaccine and experienced lower rates of influenza infection, reinforcing the vaccine’s role in preventing illness during seasonal outbreaks.

The research, led by Christopher M. Worsham and colleagues, comes amid ongoing discussions about childhood influenza vaccination policies in the United States. Earlier this year, the
U.S. Centers for Disease Control and Prevention (CDC)
removed annual influenza vaccination from its childhood immunization recommendations, citing concerns regarding the availability of recent randomized evidence and the historical reliance on observational studies to estimate vaccine effectiveness.
To address concerns about bias in traditional observational studies, researchers used a natural experiment based on children's birth months. Because annual flu vaccines become available in mid-August and young children often attend wellness visits around their birthdays, those born in the fall are more likely to receive the vaccine than children with summer birthdays.
Using this variation, the team analyzed data from five influenza seasons between 2016 and 2023 (excluding the COVID-19–affected 2020–21 and 2021–22 seasons) and compared vaccination rates and influenza outcomes among children aged 2–5 years.

  • Children born in the fall were more likely to receive the influenza vaccine than those born during the summer.
  • During the 2022–23 influenza season, 50.8% of children aged 2–5 years born between September and November were vaccinated, compared with 38.3% of those born between June and August.

  • The estimated number of influenza cases prevented ranged from 9.3 per 100 vaccinated children in the 2016–17 season to 14.3 per 100 vaccinated children in the 2019–20 season.
  • The protective effect of influenza vaccination was observed consistently across multiple influenza seasons.
The authors noted that the study design offers a practical way to evaluate vaccine effectiveness using real-world data without requiring large, costly randomized clinical trials each year. By leveraging the random timing of birth months, the analysis reduces the likelihood that differences in health behaviors or healthcare access explain the observed results.
Overall, the study provides contemporary evidence that annual influenza vaccination lowers the risk of influenza infection in young children. The researchers suggest that these findings may help inform ongoing discussions regarding childhood influenza vaccination strategies and the value of maintaining preventive measures against a disease that continues to cause substantial illness and pediatric deaths each year.
