Platelet-Rich Fibrin Plus Minocycline Shows Comparable Short-Term Outcomes to Arestin in Periodontal Treatment: Study
USA: Researchers have found in a new study that platelet-rich fibrin (PRF) combined with minocycline demonstrated comparable short-term outcomes to Arestin as an adjunct to non-surgical periodontal therapy (NSPT). Treatment selection may therefore depend on clinician preference, patient factors, cost, availability, and ease of handling. Larger randomized controlled trials with longer follow-up are needed to confirm these findings.
The study, published in Oral Health and Preventive Dentistry, was conducted by Nathan E. Estrin, Adjunct Professor at the University of Iowa College of Dentistry and Dental Clinics, Iowa City, Iowa, USA, and colleagues. The researchers compared the clinical effectiveness of minocycline delivered through a PRF scaffold with Arestin, a commercially available formulation containing 1 mg of minocycline microspheres, in patients with periodontitis.
Periodontitis is a chronic inflammatory condition affecting the tissues supporting the teeth. Although NSPT is the standard initial approach to treatment, some periodontal pockets may require additional interventions to control inflammation and improve clinical outcomes. PRF has emerged as a potential local drug-delivery scaffold that could help deliver therapeutic agents directly to affected sites.
The split-mouth randomized controlled clinical trial included 15 participants, of whom 13 completed the six-month follow-up. Each participant contributed two interproximal periodontal sites with pocket depths of 4–6 mm. Following NSPT, one site received PRF combined with minocycline, while the other was treated with Arestin. The researchers assessed probing depth (PD), clinical attachment level (CAL), gingival recession (GR) and bleeding on probing (BOP) at baseline and after six months.
The findings revealed the following:
- Reduction in probing depth: Mean PD decreased by 1.13 ± 0.15 mm in the PRF-plus-minocycline group and 1.00 ± 0.67 mm in the Arestin group.
- Improvement in clinical attachment: CAL improved by 1.02 ± 0.66 mm with PRF plus minocycline and 1.08 ± 0.61 mm with Arestin.
- Comparable clinical outcomes: Between-group differences in PD, CAL, and GR were not statistically significant, with p-values of 0.609, 0.789, and 0.096, respectively.
- Attachment gains across treatment groups: Approximately 61.5% of sites in both groups achieved CAL gains of 1–2 mm, while 15.4% of sites had a residual PD of 3 mm at six months.
The researchers noted that both treatment approaches produced significant improvements from baseline, with neither demonstrating a statistically significant advantage over the other. However, the small sample size and limited follow-up period restrict the strength and generalisability of the findings.
The researchers concluded that PRF combined with minocycline may offer a viable alternative to Arestin® as an adjunct to NSPT, with comparable outcomes over six months. Larger randomized trials involving more participants and longer follow-up are required to establish the long-term effectiveness of both approaches.
