Perianal Disease in Ulcerative Colitis Uncommon and Usually Less Aggressive: Study

Spain: Researchers have found in a new study that perianal disease affects approximately 4% of patients with ulcerative colitis (UC) and is considerably less common than in Crohn's disease. When it occurs in UC, it generally follows a milder clinical course, with fewer complex fistulas, abscesses, and destructive lesions.

These findings suggest that perianal disease in UC is typically less aggressive, although careful evaluation remains essential, as severe or complex perianal involvement may indicate an alternative diagnosis such as Crohn's disease.
The findings were published in Inflammatory Bowel Diseases by Montserrat Guasch from the Digestive and General Surgery Department, Hospital de Viladecans, Spain, and colleagues. The researchers investigated the prevalence, clinical characteristics, and treatment requirements of perianal disease in adults with ulcerative colitis using data from the ENEIDA registry, one of Spain's largest inflammatory bowel disease databases.
Although perianal disease is a well-recognized complication of Crohn's disease, its occurrence in ulcerative colitis has been less extensively studied. To better understand its presentation and management, the investigators retrospectively analyzed demographic, clinical, and treatment data from patients with ulcerative colitis who developed perianal disease.
The study included 9,884 patients with ulcerative colitis, among whom 411 developed perianal disease, corresponding to an incidence rate of 0.5 cases per 100 person-years. Notably, 22% of patients developed perianal disease before receiving a diagnosis of ulcerative colitis, highlighting that perianal manifestations may precede intestinal symptoms in a subset of individuals.
The study revealed the following findings:
  • Anal fissures were the most common perianal lesion, occurring in 52% of patients.
  • Perianal fistulas were present in 44% of patients.
  • Perianal abscesses occurred in 39% of patients.
  • Overall, perianal disease in ulcerative colitis was less aggressive than that typically seen in Crohn's disease.
  • Male sex was associated with an increased risk of developing perianal disease.
  • The presence of extra-intestinal manifestations significantly increased the risk of perianal disease.
  • Ulcerative proctitis was associated with a lower risk of developing perianal disease.
  • Perianal surgery was required in 44% of patients.
  • Biologic therapy for perianal disease was needed in only 12% of patients.
  • Overall remission was achieved in 91% of patients, with most responding well to conservative treatment or limited surgical intervention.
The researchers concluded that perianal disease is an uncommon complication of ulcerative colitis, affecting about 4% of patients, and may precede the diagnosis of ulcerative colitis in nearly one-quarter of cases. Compared with Crohn's disease, it is generally less aggressive and can usually be managed with conservative treatment or perianal surgery, with only a minority requiring biologic therapy. The authors noted that severe or complex perianal disease should prompt consideration of an alternative diagnosis, such as Crohn's disease.
