Piretanide is an antihypertensive agent belonging to Loop Diuretics.
Piretanide is a loop diuretic used to manage essential arterial hypertension and edema cardiac, hepatic, and renal origin.
Piretanide almost completely absorbed after oral doses. Piretanide is extensively bound to plasma proteins.
Piretanide is available in the form of Oral Tablet.
Piretanide is available in India, US, Japan, Germany, France, Italy, and Canada.
Piretanide belonging to the Loop Diuretics, acts as an antihypertensive agent.
Piretanide inhibits the reabsorption of electrolytes primarily in the thick ascending limb of the loop of Henle and in the distal renal tubules. It may also have a direct effect in the proximal tubules. Excretion of sodium, potassium, Ca and chloride ions is increased, and water excretion enhanced.

Piretanide is approved for use in the following clinical indications
- Hypertension and edema associated with heart failure
Piretanide is a loop diuretic used to manage essential arterial hypertension and edema of cardiac, hepatic, and renal origin.
- Hypertension
Adult: 6-12 mg daily.
- Edema associated with heart failure
Adult: 3-6 mg daily.
Piretanide is available in the form of Oral Tablet.
Piretanide is contraindicated in patients with
- Patient with severe hepatic or renal impairment
- Patient with anuria
- Addison's disease
- Hypercalcaemia.
- Fluid and electrolyte disturbances
- hepatic cirrhosis
- Gout
- diabetes mellitus
- Elderly; severe heart failure, renal or hepatic impairment.
- Monitor blood glucose concentrations in patients taking antidiabetics.
- Monitor for signs of fluid and electrolyte imbalance.
- May exacerbate or unmask SLE.
- During pregnancy and lactation.
- Hyponatraemia, hypokalaemia, hypomagnesaemia, hypochloraemic alkalosis, increased Ca excretion, hypotension; nausea, GI disturbances, hyperuricaemia, gout; hyperglycaemia; temporary increase in plasma cholesterol and TG concentrations; rarely, rashes, photosensitivity, bone marrow depression, pancreatitis, tinnitus and deafness, muscle cramps.
- Cephalosporins
May increase the nephrotoxicity of cephalosporins.
- Aminoglycosides
May increase ototoxicity of aminoglycosides and other ototoxic drugs.
- Metolazone
Severe electrolyte disturbances may occur when used with metolazone.
- Lithium
May increase the plasma concentrations of lithium.
- NSAIDs
NSAIDs may reduce the efficacy of piretanide.
- Probenecid
Decreased natriuretic effect when used with probenecid.
