Study of 720,000 Adults Links Artificial Sweeteners to Higher Diabetes Risk - Video
Overview
A new systematic review and meta-analysis suggest that adults who consume higher amounts of artificial sweeteners may face a greater risk of developing type 2 diabetes (T2D). The findings, published in Frontiers in Nutrition, add to an ongoing debate over whether sugar substitutes are truly metabolically neutral over the long term.
Researchers analyzed six prospective cohort studies involving 721,003 adults from the US, Japan and Europe, with participants followed for an average of 10.1 years. During that period, 38,445 cases of T2D were recorded.
Compared with people reporting little or no artificial sweetener consumption, those with higher intake had a 31% higher relative risk of developing T2D. The analysis included major cohorts such as NutriNet-Santé, EPIC-InterAct, MESA, the Japanese JPHC study, the Health Professionals Follow-up Study, and the Nurses’ Health Studies.
However, the researchers stressed that the findings do not prove artificial sweeteners cause diabetes. All included studies were observational, meaning other factors could partly explain the association. Artificial sweetener consumption may also be more common among people who already have obesity or other metabolic risk factors.
The researchers pointed to several possible biological explanations, including changes in the gut microbiome, insulin sensitivity, intestinal barrier function, inflammation, and pathways involved in appetite and energy regulation.
Importantly, the certainty of evidence was rated low, partly because only six studies were available and results varied substantially between them.
The findings therefore do not mean that artificial sweeteners should automatically be avoided. Instead, they highlight the need for more long-term randomized research to determine whether replacing sugar with artificial sweeteners actually improves metabolic health.
For now, the bigger takeaway may be simple: swapping sugar for sweetness alone may not be enough—overall diet quality still matters.
