Nearly 80 Percent of Instagram Smoking Posts Promote Positive Sentiment Toward Tobacco Use: Study

A recent cross-sectional analysis published in the Indian Journal of Community Medicine reveals that modern social media aggressively glamorizes tobacco use, with nearly 80% of evaluated posts promoting a positive sentiment towards smoking. This rampant digital normalization, fueled by high user engagement and covert brand marketing, poses a critical challenge for contemporary public health and clinical interventions.
Despite strict regulations on traditional tobacco advertising, social media remains an unmonitored space that normalizes youth substance use. To address this public health gap, Dr. Sonal Bhatia and colleagues at the Maulana Azad Institute of Dental Sciences conducted a content analysis of Instagram posts to identify smoking imagery, assess user engagement, and evaluate the content's underlying intent.
Therefore, the researchers conducted a cross-sectional analysis of 750 public Instagram posts from India (February–May 2023) to evaluate digital tobacco smoking imagery. After excluding smokeless and non-tobacco content, the study utilized an eight-domain qualitative codebook to assess key endpoints: user engagement, sentiment, explicit tobacco use, smoke play, brand promotion, and the presence of health warnings.
Key Clinical Findings of the Study Includes:
High Public Engagement: The analysis revealed substantial platform visibility, with posts garnering an average of 361.8 likes and 12.7 comments from users averaging 17,262 followers.
Dominance of Positive Sentiment: Researchers found that an overwhelming 79.7% of the imagery conveyed a positive sentiment toward smoking, whereas a mere 0.9% depicted negative or anti-smoking perspectives.
Prevalence of Smoking Artifacts: Investigators noted artifacts in 75.6% of posts, with traditional cigarettes being the most common (61.2%), followed by electronic nicotine delivery systems (ENDS) (17.9%) and marijuana (8.2%).
Covert Brand Promotion: The evaluation identified explicit or organic brand promotion in 22.3% of the sampled posts, effectively evading existing platform policies and regulatory bans.
Absence of Health Warnings: Scientists highlighted a concerning lack of risk communication, as health warnings were present in only 2.8% of the posts and anti-smoking awareness intent in just 1.5%.
The results suggest rampant glamorization and covert marketing of smoking on Instagram, with 79.7% of posts reflecting a positive sentiment and nearly 62% showing explicit tobacco use. This aesthetic portrayal poses a substantial threat to public health efforts aimed at de-normalizing tobacco consumption among digital audiences.
Thus, the study concludes that clinicians and public health agencies may benefit from integrating automated social media monitoring into their surveillance frameworks while advocating for transparent digital reporting mechanisms to better navigate and counteract covert online tobacco promotion.
The reliance on a single platform's algorithmic top posts and selected hashtags may have omitted some counter-narrative content, indicating that future studies could smoothly explore automated multi-platform sampling and in-depth thematic analyses to more comprehensively map the digital tobacco ecosystem.
