Most Diabetic Patients With Atypical Renal Features Have Non-Diabetic Kidney Disease, Suggests Study

A recent retrospective study published in the Indian Journal of Nephrology in February 2026 reveals that 60.79% of diabetic patients with atypical renal features actually suffer from isolated non-diabetic kidney disease, unlocking entirely new and potentially reversible treatment pathways.
Non-diabetic kidney disease (NDKD) is frequently missed in diabetic patients due to a reluctance to perform renal biopsies. To address this diagnostic gap, Dr. Shyam Bihari Bansal and his team at Medanta analyzed the clinicopathological spectrum of kidney biopsies in diabetics presenting with atypical renal features.
Therefore, the retrospective study of 130 diabetic patients who underwent kidney biopsies at Medanta Hospital (2013–2022) evaluated the clinical presentations, prevalence, and histopathology of NDKD, DKD, and combined disease, excluding those with malignancies or insufficient tissue.
Key Clinical Findings of the Study Includes:
• High NDKD Prevalence: Researchers reported that an overwhelming 60.79% of the biopsied patients exhibited isolated NDKD, compared to a much lower 26.15% diagnosed with isolated DKD and 13.07% facing a combined disease state.
• Retinopathy Differentiator: The study highlighted that diabetic retinopathy was drastically less common in patients with NDKD (8.8%) versus those with isolated DKD (58.8%), proving that the absence of eye complications is a highly useful clinical discriminator.
• Primary Clinical Triggers: Investigators noted that recent-onset nephrotic syndrome was the most frequent clinical presentation driving biopsy decisions (36.92%), followed closely by acute kidney injury and rapidly progressive renal failure.
• Microscopic Hematuria Marker: The thorough analysis revealed that microscopic hematuria was notably higher in the NDKD patient group (37.97%) than in either the isolated DKD (14.7%) or combined disease cohorts.
• Predominant Histological Lesions: Findings demonstrated that membranous nephropathy (13.92%) and IgA nephropathy (12.66%) were the most prevalent NDKD glomerular lesions, which are crucial discoveries because they present actionable targets for non-diabetic medicinal therapies.
The results suggest that NDKD is remarkably prevalent, safely observed in 60.79% of diabetic individuals subjected to renal biopsy, thereby reinforcing the immense clinical value of definitive histopathological evaluation.
Thus, the study concludes clinicians may find it highly advantageous to thoughtfully maintain a low threshold for performing kidney biopsies in diabetic patients who present with atypical clinical features, as identifying potentially reversible non-diabetic conditions can gently guide more targeted management and steadily improve overall patient prognosis.
Although the single-center, retrospective nature of the study and the absence of comprehensive long-term follow-up data regarding patient outcomes stand as limitations, future multicenter longitudinal studies could delicately expand upon these insights to further clarify therapeutic trajectories without minimizing current findings.
