Bahraich: A major controversy has erupted in Payagpur, a town in Uttar Pradesh’s Bahraich district, after a criminal case was filed against two prominent individuals — the Chairman of the Payagpur Nagar Panchayat and the former Superintendent of the Community Health Centre (CHC). The case centres around the alleged forging of a medical certificate aimed at derailing a government-led inquiry.
The controversy dates back to 2023, when the Uttar Pradesh state government issued the Payagpur Nagar Panchayat a notice after it was found guilty in multiple complaints. As per officials, instead of responding to the inquiry, he attempted to sidestep the process by obtaining a fabricated medical certificate that falsely claimed he was medically unfit for a month.

The certificate in question was issued on the official letterhead of CHC Payagpur and included a fabricated prescription and a recommendation for one month of bed rest. However, officials found discrepancies in the document, prompting the commissioner to initiate an investigation.
The inquiry, headed by CDO Mukesh Chandra and chief medical officer (CMO) Dr Sanjay Sharma, confirmed the medical report to be a forgery. The CMO subsequently filed a complaint with the Payagpur police on Saturday, triggering criminal proceedings against both the Panchayat and the doctor.

An FIR has been lodged against both the accused under sections 61, 319(2), 338, 336(3), and 340(2) of the BNS. The CMO said in his complaint that Balendu Srivastava committed this act with the intention of affecting the development work of the Nagar Panchayat, reports Bhaskar.
According to TOI, during the probe, further discrepancies came to light. Dr Sharma stated that Srivastava's name was not found in the OPD register at the CHC. Additionally, Dr Soni allegedly created a new register solely to insert Srivastava's name. Moreover, upon checking the original medical slip, it was found to be issued in the name of a woman, not Srivastava.
Legal proceedings are now underway, and authorities have confirmed that further administrative actions may be taken as the investigation progresses.
