Pregnancy Pillows Dramatically Ease Third Trimester Fatigue, Study Finds

Is there a simple, non-pharmacological intervention that can meaningfully ease the discomfort and fatigue so common in your pregnant patients’ third trimester? New research published in BMC Pregnancy and Childbirth suggests that the humble pregnancy pillow could offer substantial benefits for both comfort and fatigue in late pregnancy.
Study Design: A Triple-Blind Randomized Controlled Trial
This study was conducted at a university hospital in Türkiye, enrolling 77 primiparous pregnant women (39 in the intervention group and 38 in the control group). Participants were randomized to either use a specially designed pregnancy pillow or receive routine care during their third trimester. Importantly, the trial was triple-blind: participants, data collectors, and statisticians were all blinded to group assignments, enhancing the rigor of the results.
What Did the Pregnancy Pillow Involve?
The intervention group received a multi-zone pregnancy pillow, designed to support the cervical, abdominal, lumbar, and lower extremity regions. Women were instructed to use the pillow for at least 6 hours nightly, primarily in the left lateral position, for 8 weeks. Pillow materials were hypoallergenic and easy to clean, and no adverse effects were reported.
Key Outcomes: Fatigue and Comfort
Researchers measured outcomes using the Chalder Fatigue Scale (CFS) and the Prenatal Comfort Scale (PCS). After 8 weeks:
Fatigue scores dropped sharply in the pillow group (from 19.4 to 7.2), while the control group’s fatigue increased (from 14.8 to 28.8).
Comfort scores increased in the pillow group (from 53.2 to 69.2) but decreased in the control group (from 60.5 to 40.6).
The pillow group slept longer on average and reported better overall well-being.
Why Does This Matter?
Fatigue and discomfort in late pregnancy are linked to poorer maternal adaptation, higher risks of operative delivery, and postpartum depression. This study is among the first RCTs to demonstrate that a low-risk, simple intervention like a pregnancy pillow can reduce clinically significant symptoms during the third trimester without side effects.
