Morning Immunotherapy May Improve Survival Outcomes in Advanced Cancer Patients: Meta-Analysis Finds
Austria: A new study has revealed that administering immune checkpoint inhibitors (ICIs) earlier in the day was associated with improved overall survival and progression-free survival in patients with advanced cancers. The benefits were particularly notable in non-small cell lung cancer, gastric cancer, renal cell carcinoma, small cell lung cancer, and biliary tract cancer.
The findings were published in JAMA Network Open by Shota Inoue and colleagues from the Department of Urology at the Medical University of Vienna.
Given the role of circadian rhythms in regulating immune function, the researchers investigated whether the timing of immunotherapy administration influences outcomes in patients with advanced solid tumors.
For this purpose, they conducted a systematic review and meta-analysis comparing early versus late ICI administration. The analysis included randomized clinical trials and prospective and retrospective cohort studies identified through MEDLINE, Embase, and Web of Science Core Collection searches in February 2026.
The primary endpoints were overall survival (OS) and progression-free survival (PFS), assessed using hazard ratios with 95% confidence intervals.
Overall, 29 studies involving 6,129 patients were included. These comprised one randomized clinical trial in non-small cell lung cancer, one prospective cohort study in head and neck squamous cell carcinoma, and 27 retrospective cohort studies across multiple cancer types, including melanoma, gastric cancer, renal cell carcinoma, esophageal cancer, urothelial carcinoma, hepatocellular carcinoma, biliary tract cancer, and small cell lung cancer.
The key findings were as follows:
- Earlier administration of ICIs was associated with significantly improved clinical outcomes compared with later administration.
- Patients receiving immunotherapy earlier in the day had a 40% lower risk of death than those treated later.
- Earlier ICI administration was also linked to a 38% lower risk of disease progression.
- Improved overall survival and progression-free survival with earlier immunotherapy timing were observed in patients with non-small cell lung cancer.
- Patients with gastric cancer receiving earlier ICI administration also demonstrated better survival outcomes.
- Earlier immunotherapy administration was associated with improved overall survival and progression-free survival in renal cell carcinoma.
- Similar benefits were observed among patients with small cell lung cancer treated earlier in the day.
- Patients with biliary tract cancer also showed improved survival outcomes with earlier immunotherapy administration.
The authors suggested that immunotherapy timing may be a modifiable factor influencing treatment effectiveness, potentially due to circadian variations in immune activity.
However, they noted that most included studies were retrospective, increasing the risk of selection bias and unmeasured confounding factors. Substantial heterogeneity across studies also suggested that the impact of treatment timing may differ by cancer type, patient characteristics, and the specific ICI used.
The researchers further highlighted the lack of standardized definitions for early and late administration timing, while inconsistent adverse event reporting limited safety analysis.
The authors concluded that earlier immunotherapy administration was associated with improved survival outcomes in advanced cancers and emphasized the need for prospective multicenter randomized trials to confirm the findings and establish standardized timing strategies.
