Modified Mattress Suture Enables Immediate IOP Control with Baerveldt Implant, Suggests Study

A recent retrospective cohort study published in the Journal of Current Glaucoma Practice in July 2026 shows that a simple, reversible mattress suture modification to the Baerveldt glaucoma implant (BGI) safely provides immediate eye pressure control. By bypassing the standard six-to-eight-week delay, this technique achieved a 9.5 mm Hg lower mean intraocular pressure (IOP) on postoperative day one.

While standard non-valved glaucoma shunts require a six-to-eight-week temporary suture ligation to prevent hypotony, this delay limits their utility for eyes requiring immediate pressure reduction, a clinical gap previously addressed by short-term trials of a ventilating suture Baerveldt glaucoma implant (vsBGI). Building on this work, Quinton H. Christensen and colleagues from the University of Iowa Carver College of Medicine and Department of Ophthalmology and Visual Sciences compared the safety and clinical efficacy of the vsBGI against the standard unmodified implant over a three-month postoperative period.

Therefore, the retrospective cohort study at the University of Iowa compared 62 eyes receiving the modified implant with 62 eyes receiving the standard device between 2016 and 2023, utilizing linear and Poisson mixed-effects models. Excluding patients under 18 years of age, the investigation evaluated primary endpoints of IOP, medication burden, and visual acuity, alongside secondary outcomes of safety and cup-to-disk ratio (CDR).

Key Clinical Findings of the Study Includes:

  • Rapid IOP Control: Study findings showed the modified group achieved significantly lower eye pressure than the standard group at day one and week one, with mean differences of 9.5 mm Hg and 5.8 mm Hg, respectively (both p < 0.001).

  • Reduced Medication Burden: Trial results showed the modified group required significantly fewer medications from week one through month three, with mean differences of 0.97 at week one (p < 0.001) and 0.31 at month three (p = 0.017).

  • Equivalent Mid-Term Pressures: Study comparisons demonstrated no significant IOP difference between groups at month one (mean difference 2.4 mm Hg, p = 0.083) and month three (mean difference -1.7 mm Hg, p = 0.241).

  • Similar Safety Profiles: Trial complications were identical, with 19 eyes (31%) in each group experiencing at least one adverse event (p = 1.000) and both reporting a 4.8% incidence of clinically significant hypotony.

  • Stable Visual and Structural Metrics: Study follow-up showed no significant differences in visual acuity (all p ≥ 0.081) or CDR (p = 0.514) at any time point up to month three.

The results suggest that the reversible ventilating suture modification for the BGI provides immediate postoperative eye pressure control—demonstrated by a 9.5 mm Hg lower mean IOP at day one—while successfully reducing the three-month medication burden without increasing the 31% complication rate.

The surgical modification offers ophthalmologists a practical, reversible technique to achieve immediate pressure control in patients with advanced glaucoma or highly elevated pre-surgical pressures, helping to protect against progressive vision loss during the standard device's delay period.

Although the retrospective nature of this analysis and its brief three-month follow-up present limitations regarding long-term outcomes and surgeon selection bias, future prospective, randomized trials will be essential to fully establish the procedure's long-term durability and comparative performance.
