PRP Injection May Promote Healing of Damaged Spinal Discs: Case Report

A recent case report reveals that a simple, minimally invasive platelet-rich plasma (PRP) injection doesn't just mask chronic lower back pain-it actually heals the underlying structural damage within the spinal disc.
These findings of the case report are published in the Indian Journal of Pain in July 2026 by Anurag Agarwal and colleagues from the Department of Anaesthesia and Pain Medicine.
The documented case holds significant weight for modern clinical practice because standard conservative protocols, including extended pharmacotherapy and physical therapy, frequently fail to resolve discogenic pain, while traditional surgical options like spinal fusion carry substantial inherent risks of infection or implant failure. By successfully utilizing a targeted biological approach, treating clinicians can offer a disease-modifying alternative that directly addresses the root intervertebral pathology rather than merely masking the patient's ongoing chronic pain symptoms.
A thirty-year-old female suffering from highly debilitating axial low back pain for eight consecutive months underwent advanced magnetic resonance imaging (MRI) and provocative discography, which definitively confirmed an L4-L5 intervertebral disc injury characterized by a high-intensity zone. Following a carefully guided intradiscal PRP injection, the patient experienced a remarkable reduction in functional disability, with subsequent radiological imaging at the one-year mark confirming the complete resolution of the structural annular tear alongside sustained clinical improvement.
Biologically, localized annular tears permit inflammatory mediators to infiltrate normally avascular and aneural intervertebral disc spaces, thereby undesirably sensitizing peripheral nociceptors and generating persistent chronic pain for the affected individual. Introducing concentrated autologous growth factors directly into this region via PRP helps neutralize the localized inflammation, aggressively stimulates chondrogenesis within the damaged disc cells, and actively drives the necessary structural repair of the compromised connective tissue.
For active pain management practitioners, this successful intervention highlights the absolute necessity of utilizing objective advanced diagnostic tools, like controlled provocative discography, to precisely identify eligible candidates who might substantially benefit from regenerative biological therapies. While this non-surgical treatment shows tremendous promise for elevating overall patient care, attending physicians must carefully conduct further randomized controlled trials to establish optimal biological dosing strategies and standardized long-term therapeutic protocols.
