Giving Children a Voice: The Evolution of Pediatric Perioperative Care
When children undergo surgery, the clinical outcome is only part of the story. Their emotional journey—everything from anxiety to hunger and confusion—shapes how they remember the experience. Until now, healthcare teams have lacked a reliable way to capture these unique perspectives directly from young patients.
A new study published in Anesthesiology introduces EVANpedia, a groundbreaking self-reported questionnaire tailored to assess perioperative experiences in children and adolescents.
Why Kids Need Their Own Experience Survey
While patient-reported outcome measures are a staple for adult surgical care, children’s needs and perceptions are fundamentally different. Existing tools often miss the mark with young patients, who may not understand adult-centric language or priorities. The research team behind EVANpedia recognized that children’s voices must be heard—on their own terms.
How EVANpedia Was Born
The researchers embarked on a two-phase, multicenter journey involving 600 patients aged 7 to 17:
Phase 1: In-depth interviews and pre-testing helped build an age-appropriate item pool reflecting real children’s concerns after surgery.
Phase 2: Rigorous psychometric validation trimmed the questionnaire to 26 items grouped into six dimensions:
Anxiety
Thirst and Hunger
Loss of Autonomy
Pain
Nausea and Vomiting
Confusion
Children completed the questionnaire within 24 hours post-surgery, sharing how they truly felt—without parental interpretation.
What Did the Study Find?
EVANpedia is reliable and valid: Children’s responses showed strong consistency and accurately reflected their experiences.
Most challenging aspects: Thirst and hunger scored lowest, highlighting the impact of fasting and delayed surgery times. Nausea and vomiting scored highest, indicating most children felt less bothered by these symptoms.
Emotional well-being matters: 9% of children expressed disappointment with their surgical experience, scoring notably lower on all dimensions.
Sensitive to real-life changes: The tool detected differences based on gender, age, surgery type, timing, and pain or anxiety levels.
Why This Matters for Future Care
By capturing the emotional and physical realities of pediatric surgery straight from the children themselves, EVANpedia helps clinicians design more empathetic and effective care pathways. It also offers a robust, child-centered outcome for research and hospital quality improvement.
