USA: A Scientific Statement from the American Heart Association has revealed that moderate caffeine and coffee consumption is generally well tolerated and safe for most adults. Coffee may also offer potential benefits in conditions such as coronary heart disease, stroke, heart failure, atrial fibrillation, hypertension, and type 2 diabetes, although these associations are not yet definitive. The evidence is based mainly on large observational cohort studies, with limited data from small randomized trials.

The scientific statement, published in Circulation, was led by Gregory M. Marcus, professor of medicine at the University of California, San Francisco School of Medicine, and colleagues. The review examined the latest evidence on caffeine and cardiovascular health, highlighting both its potential benefits and risks while emphasizing that the source and amount of caffeine, as well as individual differences in metabolism, influence its effects.
Caffeine is the world's most widely consumed psychoactive substance, with coffee serving as the primary source for most adults. Tea, chocolate, soft drinks, energy drinks, and certain over-the-counter and prescription medications also contribute to daily caffeine intake. According to the statement, consuming up to 400 mg of caffeine per day—roughly equivalent to 3–5 cups of 8-ounce black coffee—appears safe for most healthy adults and may be incorporated into a heart-healthy lifestyle.

  • Coffee consumed without added sugar, flavored syrups, cream, or other high-calorie additives is more likely to retain its potential health benefits.
The review also noted that caffeine's effects differ between acute and habitual consumption. Shortly after intake, caffeine may temporarily increase blood pressure, heart rate, blood glucose levels, and alertness. Some people may also experience palpitations, anxiety, or sleep disturbances. However, many habitual coffee drinkers develop partial tolerance to these short-term effects over time.
The authors emphasized that individuals metabolize caffeine differently because of factors such as genetics, age, medications, underlying health conditions, and previous caffeine exposure. As a result, the same amount of caffeine may be well tolerated in one person but produce noticeable side effects in another.
The statement also cautioned against directly attributing coffee's apparent health benefits solely to caffeine. Coffee contains several biologically active compounds, including polyphenols with antioxidant and anti-inflammatory properties, which may contribute to the favorable cardiovascular associations observed in epidemiological studies. Similarly, findings from coffee research should not be extrapolated to synthetic caffeine supplements or high-caffeine energy products, which have shown a less favorable safety profile.
The authors acknowledged that much of the current evidence comes from observational research, limiting the ability to establish cause-and-effect relationships. They also noted the possibility of "healthy user bias," in which moderate coffee drinkers may engage in other healthy lifestyle behaviors that influence cardiovascular outcomes.
The researchers called for more randomized controlled trials to clarify the independent effects of caffeine and other coffee components, determine optimal intake levels in people with cardiovascular disease or hypertension, evaluate different caffeine sources, and better understand why responses vary among individuals.
Overall, the American Heart Association concluded that moderate coffee consumption can be part of a healthy lifestyle for most adults, while advising caution with high-dose caffeine products such as energy drinks. The authors stressed that caffeine recommendations should be individualized based on a person's health status, medications, and tolerance rather than applying a single approach to everyone.
