Periodontally Accelerated Osteogenic Orthodontics (PAOO) reduces treatment duration to just one-fourth of the standard timeframe while significantly enhancing long-term stability and alveolar bone volume by integrating piezoelectric decortication and bone grafting with conventional mechanics.

These findings are published in the Indian Journal of Dental Sciences by lead author Dr. Rupali Ravindra Borade and a team of researchers from the Departments of Periodontology and Orthodontics at the SMBT Dental College and Hospital in February 2026.

Although this technique, formally identified as Periodontally Accelerated Osteogenic Orthodontics (PAOO), represents a significant breakthrough for clinical practice because it enables substantial tooth movement without being constrained by a patient's preexisting alveolar bone volume. It serves as a vital resource for adult orthodontics, as it not only expedites the treatment process but also improves the overall structural integrity of the periodontium and repairs preexisting defects like fenestrations or dehiscences

A 28-year-old male with significant malalignment and bimaxillary protrusion underwent thorough general assessments and routine laboratory investigations before receiving a treatment plan consisting of fixed orthodontic mechanotherapy paired with surgical piezoelectric decortication and bone grafting. Following full-thickness flap elevation and selective cortical perforations, demineralized freeze-dried bone allograft (DFDBA) was placed, and orthodontic forces were immediately activated, leading to a six-month follow-up that confirmed stable probing depths of 3 millimeters or less, maintained pulpal vitality, and a total absence of root resorption.

From a biological perspective, the effectiveness of this procedure is driven by the induction of the Regional Acceleratory Phenomenon (RAP), which is a localized healing response characterized by transient bursts of hard and soft tissue remodeling. This process creates a temporary osteoporotic state that decreases regional bone density and accelerates bone turnover, effectively allowing teeth to move more rapidly through the alveolar structure than through standard mechanical force alone.

Clinical Relevance and Potential Implementations

The clinical relevance of this approach lies in the integration of piezosurgery, which offers practitioners greater surgical control and precision compared to traditional drills while minimizing trauma to anatomical structures and enhancing patient comfort. By augmenting bone volume and improving the gingival biotype, this methodology provides a predictable and efficient solution for complex orthodontic cases, effectively bridging the gap between periodontal health and aesthetic alignment for modern dental patients.
