Metabolic Syndrome Linked to Accelerated Brain Aging, Finds Study

Sweden: Middle-aged and older adults with metabolic syndrome had significantly older-appearing brains than those without the condition, according to a large U.K. Biobank imaging study. Participants with metabolic syndrome had a higher brain age gap, indicating that their MRI-estimated brain age exceeded their chronological age. The findings suggest that metabolic syndrome—including hyperglycemia, hypertension, central obesity, low HDL cholesterol, and elevated triglycerides—may be a modifiable target for slowing brain aging and potentially reducing the risk of cognitive decline.
The study, published in Alzheimer's & Dementia, was conducted by Abigail Dove from the Aging Research Center, Department of Neurobiology, Care Sciences and Society, Karolinska Institutet, Stockholm, Sweden, and colleagues.
Metabolic syndrome (MetS), a cluster of metabolic abnormalities that raises the risk of cardiovascular disease and type 2 diabetes, has also been linked to dementia. However, its role in brain aging and the biological pathways involved have remained uncertain. To investigate this, the researchers examined data from 27,375 UK Biobank participants aged 40 to 70 years.
Metabolic syndrome was defined by the presence of at least three of five components: central obesity, hypertension, hyperglycemia, elevated triglycerides, and low HDL cholesterol. The researchers analyzed 33 plasma metabolites and estimated brain age using a machine-learning model based on 1,079 MRI brain features. The primary outcome was the brain age gap (BAG), calculated as the difference between MRI-predicted brain age and chronological age.
The study led to the following findings:
- Participants with metabolic syndrome had a significantly higher brain age gap than those without the condition, indicating accelerated brain aging.
- Brain age increased progressively with the number of metabolic syndrome components, demonstrating a dose-dependent association.
- Each of the five components of metabolic syndrome—central obesity, hypertension, hyperglycemia, elevated triglycerides, and low HDL cholesterol—was independently linked to an older-appearing brain.
- The researchers identified eight circulating metabolites that partially mediated the association between metabolic syndrome and accelerated brain aging, accounting for 2.6% to 16.5% of the relationship.
- The key mediating metabolites were related to inflammation, apolipoproteins, and fatty acid metabolism, suggesting these biological pathways may contribute to the brain-aging effects of metabolic syndrome.
The authors acknowledged several limitations. The UK Biobank cohort is healthier and less ethnically diverse than the general population, limiting generalizability. Including only participants with MRI and metabolite data may have underestimated the associations. Additionally, the cross-sectional design and single MRI assessment precluded conclusions about causality or brain aging over time.
The researchers concluded that metabolic syndrome is associated with accelerated brain aging, with inflammation and altered lipid metabolism appearing to play a partial role. They suggested that preventing or effectively managing metabolic syndrome and its individual components may offer a promising strategy to support healthy brain aging and potentially lower the risk of future cognitive decline.
