Targeting the Dorsal Root Ganglia May Transform Chronic Pain Management, Review Suggests

A recent comprehensive clinical review published in the Indian Journal of Pain in March 2026 reveals that targeted dorsal root ganglia (DRG) therapies, such as radiofrequency ablation and neuromodulation, are revolutionizing chronic pain management. These innovative interventions offer highly promising relief for complex, hard-to-treat conditions like neuropathy and radiculopathy.

While the DRG functions as a crucial sensory relay station, historically, conventional treatments like spinal cord stimulation have sometimes failed to address specific refractory neuropathic pain states, highlighting a significant clinical gap in achieving localized dermatomal analgesia. Bridging this divide, Dr. Prateek Arora and colleagues from the All India Institute of Medical Sciences in Nagpur, Maharashtra, set out to comprehensively review the fundamental neurobiology of the DRG and evaluate the efficacy of current targeted interventional strategies to solidify its role as an active therapeutic target.

Therefore, the clinical review analyzed multiple trials of targeted dorsal root ganglia (DRG) therapies, including radiofrequency ablation and neuromodulation, for chronic pain. The study focused on assessing procedural safety and the percentage of patient pain relief achieved at three to six months.

Key Clinical Findings of the Review Includes:

  • Pulsed Radiofrequency Efficacy: Researchers found that PRF for lumbar radicular pain resulted in 82% of patients reporting significant pain relief at three months, demonstrating high short-term efficacy without permanent neural damage compared to conventional methods.

  • Conventional Thermal Ablation: Investigations revealed that CRF provided sustained benefits for conditions requiring permanent neural disruption, with 65% of intercostal neuralgia patients experiencing greater than 50% pain relief at six months.

  • Neuromodulation Precision: Analysis indicated that DRG neuromodulation offers highly focused dermatomal analgesia, successfully treating complex regional pain syndrome even in cases where traditional spinal cord stimulation previously failed.

  • Innovative Ozone Therapy: The review highlighted emerging evidence that oxygen-ozone mixtures injected near the DRG successfully reduced inflammation and neural sensitization, offering substantial relief for refractory post-herpetic neuralgia.

The results suggest that the dorsal root ganglia are not merely passive conduits for sensory signals but are central, highly modifiable structures where targeted interventions like neuromodulation and radiofrequency lesioning can successfully yield significant pain reduction for a majority of chronic pain patients.

Thus, the review concludes that clinicians might consider incorporating these localized DRG-targeted interventions into their pain management algorithms, particularly for patients who have proven unresponsive to standard systemic analgesics or conventional generalized spinal stimulation.

While these targeted approaches show robust therapeutic promise, the inherent variability in procedural techniques and patient anatomy poses certain limitations in standardizing treatment protocols. Consequently, there remains a gentle but vital need for larger, well-designed longitudinal trials to fully optimize radiofrequency settings and elucidate the long-term safety profile of emerging modalities like ozone therapy.
