Severe Periodontitis Associated With Increased Ischemic Stroke Risk, finds research

A new systematic review and meta-analysis published in the Journal of Periodontology revealed severe periodontitis to be significantly associated with a higher risk of ischemic stroke, with evidence of a severity-related relationship across studies. The association was stronger in studies using standardized diagnostic criteria and longer follow-up. These findings suggest that advanced periodontitis may represent a vascular risk factor, highlighting the importance of early diagnosis, periodontal treatment, patient education, and closer collaboration between dental and medical professionals.
Periodontitis is a chronic inflammatory disease that damages the tissues supporting the teeth. Beyond tooth loss and oral complications, persistent inflammation associated with advanced gum disease has increasingly been linked to systemic conditions, including cardiovascular disease. Studies believe inflammatory mediators and vascular changes may help explain the connection, although they emphasize that the review demonstrates an association rather than direct causation.
The study reviewed 15 high-quality studies published between 2000 and 2026, including seven cohort studies and 8 case-control studies, to assess whether the severity of periodontitis influences the likelihood of developing ischemic stroke. This research followed the PRISMA 2020 guidelines and analyzed data from PubMed, Web of Science, and Scopus, focusing on studies that used clinical or radiographic diagnosis of periodontitis and clearly defined stroke outcomes.
The individuals with periodontitis had a 24% higher risk of ischemic stroke in cohort studies, while case-control studies showed that affected individuals were more than 4 times as likely to experience the condition when compared to those without periodontal disease.
The patients with severe periodontitis demonstrated more than 4 times the odds of ischemic stroke, substantially higher than those with moderate disease, who showed a more modest increase in risk. This severity-related trend strengthens the evidence that worsening gum disease may contribute to vascular damage over time.
The analysis also found that studies using modern standardized diagnostic systems, including the American Academy of Periodontology/European Federation of Periodontology (AAP/EFP) and CDC/AAP classifications, reported even stronger associations. Long-term follow-up studies lasting 15 years or more also recorded greater stroke risk, supporting the possibility that chronic inflammation from untreated gum disease may gradually increase susceptibility to ischemic stroke.
Overall, the findings highlight the importance of early diagnosis and effective management of periodontal disease, particularly before it progresses to advanced stages. This research suggest that severe periodontitis should be recognized as a probable vascular risk factor and incorporated into broader cardiovascular risk assessments.
