Abrocitinib Achieves Superior Disease Control in Atopic Dermatitis, Suggests Study
USA: Researchers have found in a new study that treatment with abrocitinib enabled more patients with atopic dermatitis to achieve minimal disease activity by week 16, including at least 90% improvement in Eczema Area and Severity Index (EASI-90) and little to no itching (Peak Pruritus Numerical Rating Scale 0–1), compared with dupilumab or placebo. The findings highlight abrocitinib's superior effectiveness in achieving near-complete skin clearance and itch relief.
The findings, published in Dermatology and Therapy, are based on a pooled post hoc analysis led by Stephen Weidinger, Department of Dermatology and Allergy, University Hospital Schleswig-Holstein, Campus Kiel, Germany, and colleagues.
Atopic dermatitis is a chronic inflammatory skin disease characterized by persistent itching, skin inflammation, and recurrent flares that can significantly impair quality of life. Achieving minimal disease activity—defined as near-complete skin clearance together with an itch-free or nearly itch-free state—is increasingly recognized as an important treatment goal. Abrocitinib, an oral Janus kinase (JAK) inhibitor, is approved for the treatment of moderate-to-severe atopic dermatitis, but data on its ability to achieve these stringent outcomes have been limited.
Researchers analyzed pooled data from the phase 3 JADE DARE, JADE COMPARE, and JADE EXTEND clinical trials. The analysis evaluated the proportion of patients achieving a composite endpoint of EASI-90 and Peak Pruritus Numerical Rating Scale (PP-NRS) 0–1 after treatment with abrocitinib 100 mg or 200 mg. The investigators also assessed outcomes in patients whose disease had responded inadequately to dupilumab.
Key findings of the study include:
- In the JADE DARE trial, 5.8% of patients receiving abrocitinib 200 mg achieved near-complete skin clearance with an itch-free state as early as week 2, compared with 1.1% of patients treated with dupilumab.
- In JADE COMPARE, by week 16, the composite endpoint was achieved by 27.1% of patients receiving abrocitinib 200 mg, 20.2% receiving abrocitinib 100 mg, 15.5% receiving dupilumab, and 5.4% receiving placebo.
- The likelihood of achieving minimal disease activity was consistent regardless of baseline itch severity.
- In the JADE EXTEND trial, patients who had responded inadequately to dupilumab also benefited from switching to abrocitinib, with 16.7% of those receiving 200 mg and 18.5% receiving 100 mg reaching the stringent composite endpoint.
The findings suggest that abrocitinib can help a substantial proportion of patients with moderate-to-severe atopic dermatitis achieve near-complete skin clearance and an itch-free state, including those with an inadequate response to dupilumab.
The researchers noted that the results are exploratory because the analysis was post hoc and lacked formal statistical testing. They added that longer-term studies are needed to further evaluate abrocitinib's efficacy and durability of response.
