GLP-1 Drugs May Increase Hair Loss Risk in Men Predisposed to Baldness: Study

USA: A Mendelian randomization study suggests that men with a genetic predisposition to androgenetic alopecia may face an increased risk of hair loss when using GLP-1 receptor agonist drugs. The findings highlight the potential role of genetic susceptibility in GLP-1-associated hair loss and warrant further research to clarify the underlying mechanisms and clinical implications.

The study, published online on September 3, 2026, in the
Journal of Investigative Dermatology
, was led by Ravi Ramessur, Center for Clinical Sciences in Dermatology, Department of Dermatology, Perelman School of Medicine, University of Pennsylvania, Philadelphia, Pennsylvania, United States, and colleagues.
GLP-1 receptor agonists (GLP-1RAs) are increasingly used to manage type 2 diabetes and obesity. Hair shedding has been reported among people taking these medications, with rapid weight loss often considered a potential explanation. However, previous observational studies have also suggested that GLP-1RA exposure may be associated with androgenetic alopecia, commonly known as patterned hair loss.
The researchers noted that changes in GLP-1 signaling could potentially influence hair follicles through several pathways. These may include direct effects on follicular biology or indirect changes involving body fat, insulin and insulin-like growth factor-1 signaling, blood pressure and testosterone. Despite these proposed mechanisms, whether GLP-1 activity itself contributes to patterned hair loss has remained uncertain.
To examine a possible causal relationship, investigators used a two-sample Mendelian randomization approach, which uses naturally occurring genetic variations as proxies for specific biological exposures. The analysis focused on the GLP1R gene, which influences the production of GLP-1 receptor proteins and can therefore serve as an indicator of GLP-1 pathway activity.
Genetic information was obtained from large publicly available databases. The eQTLGen database included data from 31,684 predominantly White men and women, while the Complex Traits Genetics group database provided information from 205,327 predominantly White men.
The key findings were as follows:
  • Genetically predicted higher GLP1R expression was associated with a 7% higher risk of androgenetic alopecia in men.
  • The 7% increased risk persisted after adjusting for hypertension, suggesting that the association was not explained by elevated blood pressure.
  • Additional analyses accounting for insulin resistance and testosterone levels also showed that the increased risk remained.
  • The findings indicate a possible biological link between increased GLP-1 receptor activity and male-pattern hair loss, beyond hair shedding related to rapid weight loss.
  • However, the study does not prove that GLP-1 receptor agonist medications directly cause hair loss in individual patients.
The researchers emphasized the need for further experimental and clinical studies to determine how GLP-1 signaling might affect hair follicles. They also highlighted the possibility that future research could examine whether genetic markers might help identify individuals who are more susceptible to hair loss during GLP-1RA treatment.
The current analysis focused on men because the genetic data and evidence for androgenetic alopecia were more robust in males. Whether similar associations exist in women remains an important question for future investigation.
