Indian Study Validates 'Rule of Halves' in Diabetes, Reveals Major Gaps in Blood Sugar Control

A recent community-based study validates the applicability of the "rule of halves" (ROH) framework to diabetes mellitus, revealing a striking 31.4% prevalence rate and highlighting a critical gap where only about 50% of treated patients achieve control, as detailed in a recent study published in the Indian Journal of Community Medicine in April 2025.
Although diabetes mellitus remains a premier public health challenge, this investigation extends the ROH concept, established for hypertension in 1972, to identify critical gaps in diabetes care; therefore, Dr. Imaad Mohammed Ismail from the Department of Community Medicine at Yenepoya Medical College aimed to assess whether this model could accurately snapshot the prevalence and treatment outcomes within a specific rural Indian population.
Therefore, the community-based cross-sectional study, conducted from March to September 2023, utilized a complete enumeration approach to evaluate 532 adults in rural Lakshmi Gudde, Karnataka, using a validated questionnaire and a United States Food and Drug Administration (FDA)-approved spot glycated hemoglobin (HbA1c) analyzer. To maintain high data integrity, the researchers excluded pregnant or terminally ill individuals while assessing primary endpoints such as disease awareness and the percentage of patients achieving adequate blood sugar control, defined as an HbA1c level of 7% or less.
Key Clinical Findings of the Study Include:
• Significant Disease Prevalence: Findings show a 31.4% prevalence rate, notably higher than the 4.3% to 25.2% range seen in prior regional research.
• Suboptimal Diagnostic Awareness: Data indicate only 63.5% of diabetic individuals were aware of their condition, exceeding the national average of 45.8% yet lagging behind the 70% awareness levels in developed nations.
• Strong Treatment Initiation: Research recorded a high treatment uptake of 88.7% among those aware, a figure superior to results from many other Indian and global cohorts.
• Disappointing Clinical Control: Results demonstrate that only 52.1% of those undergoing treatment achieved adequate blood sugar control, illustrating that nearly half remain vulnerable to chronic complications.
• Low Lifestyle Integration: Analysis revealed that only 24.5% of patients combined medication with diet and exercise, emphasizing a heavy reliance on pharmacological-only management.
The results suggest that the "rule of halves" framework is highly applicable to diabetes mellitus because, although the prevalence reached 31.4%, only approximately 52.1% of those receiving treatment successfully maintained their blood sugar levels within the target range.
Thus, the study concludes that clinicians should consider intensifying patient counseling on the critical importance of non-pharmacological lifestyle modifications to improve the overall success rate of diabetic management.
Although the study was focused on a single village and used capillary blood samples that might impact the accuracy of case classification, future investigations across diverse geographic locations could help further support the widespread relevance of this management framework.
