NMC directs medical colleges to submimt annual disclosure reports 2025 for PG courses, gives deadline

National Medical Commission
New Delhi: The Postgraduate Medical Education Board (PGMEB) of the National Medical Commission (NMC) has directed all medical colleges and institutions conducting postgraduate medical courses to submit their Annual Disclosure Report (ADR) 2025 as per the provisions of the Maintenance of Standards of Medical Education Regulations (MSMER), 2023.
The Board has specified that the report must be submitted for the period from January 1, 2025, to December 31, 2025, in respect of each PG medical course being conducted by the institutions.
Issuing a public notice on 29.5.2026 in this regard, Manish Kumar Mittal, the under secretary of PGMEB stated, "The Maintenance of Standards of Medical Education Regulations, 2023 (MSMER-2023) stipulate that, “After its establishment, every medical college or medical institution shall furnish an Annual Disclosure Report to the concerned Board, in compliance with the conditions prescribed under the notified Minimum Standards Requirements (MSRs) by UGMEB or PGMEB, and the regulations of the National Medical Commission (NMC), as applicable. Accordingly, all Postgraduate Medical Colleges/Institutions conducting PG Medical Courses are required to submit the Annual Disclosure Report (ADR)-2025 for the period from 01.01.2025 to 31.12.2025 in respect of each PG Medical Course being conducted by them."
As per the notice, the ADR will consist of two parts: Standard Assessment Form – Part A (Institutional information common to all PG specialties) and Standard Assessment Form – Part B (Specific to the concerned Broad Specialty/Super Specialty course). The required forms are available on the NMC website under the Download Application Forms section.
i. SAF-A (for institutional information) at Sr.No.2
ii. SAF-B (for specialty specific) (Broad – from Sr. No.3.1 to 3.38) (Super Specialty – from Sr. No.4.1 to 4.48)
iii. Master SAF-B Form at Sr. No.2.1 (to be used if specialty-specific SAF-B is not available)
The portal for submission of the Annual Disclosure Report by Colleges can be accessed at the following URL: https://www.nmc.org.in/annual-disclosure-report/
The Commission clarified that information related to clinical material, investigative workload and faculty attendance must be provided in various tables under column headings showing Last year/ year 3 will be of year 2025 (01.01.2025 to 31.12.2025).
It also stated that the faculty attendance Information under the column heading for the year part of the year will also be for the year 2025 (01.01.2025 to 31.12.2025) or part of it, and in the table of total eligible faculties and Senior Residents, only eligible faculties fulfilling the attendance requirement for 2025 (01.01.2025 to 31.12.2025) will be mentioned.
All medical colleges and institutes running PG medical courses have been asked to submit the relevant forms along with a fee of Rs 50,000 plus 18 per cent GST, amounting to Rs 59,000 per PG medical course. The payment must be made only through the designated payment window available on the NMC portal. No other mode of payment will be accepted.
The path for uploading of ADRs and payment of fees shall be as follows:

| Step-2 | Check the autofilled information. Upload SAF Form-A. |
| Step-3 | Upload SAF-B for each of the PG Broad/Super Specialty individually. It may be NOTED that only PDF uploaded document, which is legible and readable will be accepted. |
| Step-4 | On submission of SAF-A & B forms, option for payment of fees shall appear on portal. Payment of fees option will be opened for one transaction only. It may be noted that payment for all uploaded ADRs may be made in one-go; separate transaction for each specialty is not permissible. |
| Step-5 | On successful submission of Form/fees, a final receipt will be provided in NMC Portal for the Broad/Super Specialty. |
For technical queries related to ADR submission, institutions may contact the designated email address provided by the NMC.
Referring to this, the National Medical Commission (NMC) has written to the Directors, Principals, and Deans of all medical colleges and institutions under its purview, asking them to comply with the directions issued by PGMEB.
"Kind reference is invited to the Public Notice No. N-P011(20)/5/2026- PGMEB-NMC dated 27-05-2026 (copy enclosed) issued by the Post Graduate Medical Education Board (PGMEB) of National Medical Commission (NMC) on the above subject matter being self explanatory and inter-alia directing therein for submission of Annual Disclosure Report (ADR)-2025 for Postgraduate Medical Courses in due compliance of instructions as contained in the above said Public Notice. All the concerned Medical colleges/institutions are requested to take note of instructions in the above-referred Public Notice and ensure that the requisite admission details wrt ADR-2025 are uploaded on the portal within the timeline i.e., up to 30.06.2026 till midnight, without fail, as no extension will be granted beyond this date and forms will not be accepted physically," mentioned the letter issued by Dr. Raghav Langer NMC secretary.
