Hypoalbuminemia Drives Erectile Dysfunction in CKD, Suggests Study

A recent study published in the Indian Journal of Nephrology in June 2026 reveals a striking 55% prevalence of erectile dysfunction among non-dialysis patients, a condition that dramatically worsens as the kidney disease advances. Crucially, the research identifies hypoalbuminemia as the primary modifiable driver of this complication, offering clinicians a clear target for intervention.
Despite its profound impact on quality of life, erectile dysfunction in men with chronic kidney disease remains largely unaddressed in Indian clinical settings. To bridge this gap, Dr. Vipin Chandra and colleagues at AIIMS Patna systematically investigated the prevalence and key predictors of erectile dysfunction among local non-dialysis patients.
Therefore, to evaluate erectile dysfunction, researchers analyzed 100 sexually active male patients with chronic kidney disease (aged 25 to 50 years) using the validated IIEF-5 questionnaire. Patients with severe comorbidities, psychiatric conditions, or confounding urogenital issues were deliberately excluded to ensure accurate clinical results.
Key Clinical Findings of the Study Includes:
Prevalence Escalation: Investigators found overall erectile dysfunction impacted 55% of the cohort, progressively surging from 60% in stages I-II to 76% in stage III, peaking at 96% in advanced stages IV-V.
Severity Shift: Researchers revealed that while severe impairment was entirely absent in early disease phases, it escalated to become the predominant manifestation in advanced illness, burdening 40% of patients with stage IV and V disease.
Nutritional Influence: Authors identified hypoalbuminemia as the strongest modifiable predictor, uniquely accounting for 40% of the variance in dysfunction where every 1 g/dL increase in serum albumin yielded a 3.2-point improvement in IIEF-5 scores.
Exacerbating Factors: Scientists demonstrated that a heightened urinary albumin-to-creatinine ratio (ACR) and advancing patient age functioned as key, though less actionable, non-modifiable factors compounding the risk of compromised sexual health.
The results suggest that erectile dysfunction is exceptionally prevalent in this male demographic, worsening robustly alongside renal decline. Furthermore, malnutrition-inflammation pathways directly reflected by diminished serum albumin stand as the principal modifiable mechanisms driving this clinical complication.
Thus, the study concludes and encourages clinicians toward adopting an integrated care model that combines early routine screening using validated urological tools with prompt nutritional support to correct protein deficits, alongside optimizing proteinuria management via targeted pharmaceutical therapeutics.
While acknowledging modest constraints like a smaller cohort size and unequal distribution across disease stages, the findings highlight an inviting pathway for future research to validate albumin as an early predictive biomarker and explore novel medications for dual functional preservation.
