Ultrasound-Guided Cannulation Shows Promise for First Attempt Success: Study
Taiwan: A systematic review has found that ultrasound-guided cannulation may improve first-attempt success in haemodialysis patients, but current evidence is limited by methodological flaws, operator learning-curve effects, and clinical heterogeneity. After accounting for these limitations, there is no definitive evidence that it is superior across specific clinical or geographical settings. Therefore, well-designed, large-scale randomized trials with standardized per-patient reporting are needed to confirm its effectiveness.
The findings were published in BMC Nephrology by Tsuei-Wun Chang and colleagues, who evaluated whether ultrasound (US)-guided cannulation offers advantages over the conventional palpation-guided approach for arteriovenous access in patients receiving haemodialysis.
Reliable vascular access is essential for successful haemodialysis, and repeated cannulation can be challenging, particularly in patients with difficult-to-access fistulas or grafts. Ultrasound guidance has been increasingly adopted to improve needle placement accuracy and potentially reduce complications. However, evidence regarding its clinical benefits has remained inconsistent.
To address this question, the researchers performed a systematic review of randomized controlled trials comparing ultrasound-guided cannulation with traditional palpation-guided techniques. Five electronic databases were searched from their inception through May 2025. Quantitative analyses were conducted using pooled risk ratios and mean differences with 95% confidence intervals. Importantly, the investigators separated studies based on whether outcomes were reported for individual patients or for individual cannulation events, helping to minimize statistical bias arising from different reporting methods.
