Ultrasound-Guided Cannulation Shows Promise for First Attempt Success: Study

Taiwan: A systematic review has found that ultrasound-guided cannulation may improve first-attempt success in haemodialysis patients, but current evidence is limited by methodological flaws, operator learning-curve effects, and clinical heterogeneity. After accounting for these limitations, there is no definitive evidence that it is superior across specific clinical or geographical settings. Therefore, well-designed, large-scale randomized trials with standardized per-patient reporting are needed to confirm its effectiveness.
- The systematic review included five randomized controlled trials involving 142 unique patients and 2,894 cannulation procedures.
- Initial pooled analysis showed that ultrasound-guided cannulation modestly improved first-attempt success, with an 8% relative increase compared with conventional palpation-guided cannulation.
- When the largest study reporting outcomes by cannulation events rather than individual patients was excluded, the improvement in first-attempt success was no longer statistically significant.
- The apparent overall benefit of ultrasound guidance was primarily driven by event-based reporting studies, while patient-level analyses showed no significant advantage over conventional cannulation.
- No significant difference was observed between ultrasound-guided and palpation-guided cannulation in complication rates.
- Procedure duration could not be reliably compared because the included studies used different definitions and methods of measuring the outcome.
- The included trials showed substantial heterogeneity, particularly for first-attempt success and complication rates, owing to differences in study design, outcome reporting, and operator experience.
