CPS Mumbai releases Online Permission Forms For September, October exams, Details

CPS Mumbai releases Online Permission Forms For September, October exams, Details

Mumbai: College of Physicians & Surgeons (CPS) has released the online permission forms for CPS examination September-October 2022 which are available online till 24th June 2022 up to 4:00 p.m.

All eligible CPS enrolled students can submit permission forms online through their respective CPS login. Exam repeaters who have submitted permission forms earlier need not submit permission forms again. These students may submit examination forms directly whenever available.

CPS has asked the DMRE and DPM Students to submit separate permission forms for appearing in Part-I and Part-II examinations. Students are requested not to visit the CPS office in Mumbai for any reason due to o the pandemic phase the country is going through.

Students need to send the hard copies of all original post holding and post-completion certificates by post to the CPS office in Mumbai before 24th June 2022. FCPS students must send 3 copies of THESIS (original hard copies) to the CPS office in Mumbai before 24th June 2022.

Steps to submit online permission form -

1. Go to CPS's official website www.cpsmumbai.org.

2. Click on login to online.cpsmumbai.org

3. Students need to enter their username and password. If login is done successfully, students need to go to examination -> permission form.

4. Select your course.

5. Check all details in the form.

6. The students need to upload all post holding certificates and post-completion certificates (except the last post-completion, which needs to be submitted at the time of submission of the CPS examination form). Scanned copies of original documents have to be submitted. For FCPS course students, 6 post-holding certificates + 5 post completion certificates have to be submitted. For Diploma course students, 4 Post holding certificates + 3 post completion certificates have to be submitted.

7. Click on Submit.

8. Students have to take the printout of the form & sign it. Then take the sign of their institute head and the institution stamp and upload a scanned copy of the same in PDF format.

Students note that if they get a message "Invalid username or password," they can e-mail cpsonline@cpsmumbai.org from their e-mail id registered with CPS. It is mandatory to mention the correct enrollment no., course name, and Institute name in the e-mail.

Documents to be e-mailed –

1. Permission form (Signed and Stamped by Institution Head)

2. State Medical Council Registration / State Medical Council Registration Renewal

3. Lecture Card

4. Marksheet of DPM-PART-I (for the students who have passed the DPM Part-I examination and want to submit the permission form for DPM Part-II)

5. Marksheet of DMRE-PART-I (for the students who have passed the DMRE Part-I examination and want to submit the permission form for DMRE Part-II)

6. NOC, WORK ORDER OF DEEMED PERMANENT (for DPH course students)

7. Aadhar Card

The CPS office will process the permission form. The fee payment option will be given only when the permission form is processed. When the students get the "Pay fees" option, you may make online payment of permission form fees. You shall get the permission form fees receipt after the payment of fees.

Students need to use CPS website links to secure payment gateway websites for fee payment. Upon completing the payment transaction, they will be redirected to the CPS website with the fee receipt. Please save and print this receipt for reference. If the transaction is successful and the amount is deducted from the bank account but not redirected to the CPS website with the fee receipt, it needs to wait 1 hour. In case, please do not reinitiate payment or refresh the page to avoid duplicate payment.

Online payment mode for fees payment is the preferred option, but if any student cannot pay the fee online, they can send a Demand Draft (DD) of fees to the CPS office, Mumbai, before 24th June 2022 4:00 pm. DD of requisite fees should be drawn in favour of the "College of Physicians and Surgeons of Mumbai" payable at Mumbai. Receipt of fees by DD shall be subject to clearance of DD.

1. Diploma - Rs. 3200

2. FCPS - Rs. 13200
