CPS Notifies About Online Permission Forms For CPS Exams March-April 2024
Maharashtra: The College of Physicians and Surgeons (CPS Mumbai) has notified about online permission forms for the CPS examination in March-April 2024.
The permission forms for the CPS examination March-April 2024 are available online from 22 December 2023 to 05 January 2024 (up to 4:00 pm). All eligible CPS-enrolled students may submit permission forms online through their respective CPS login.
Exam repeaters or those who have submitted the permission form earlier, those students need not submit the permission form again. These students may directly submit examination forms whenever available. Whereas DMRE and DPM Students must submit separate permission forms to appear in Part-I and Part-II examinations. FCPS students are required to send a soft copy of THESIS to cpsonline@cpsmumbai.org and three (3) copies of THESIS (original hard copies) to CPS office Mumbai before 05-January-2024 (4:00 pm).
Steps to submit online permission form –
1. Students need to go to the official website of CPS
2. Click on Login to online.cpsmumbai.org
3. Students need to enter a username and password (If the students are getting a message "Invalid username or password", they need to e-mail cpsonline@cpsmumbai.org from their e-mail ID registered with CPS. It is mandatory to mention the correct enrollment no., course name and Institute name in the e-mail. Please do not call).
4. If login is done successfully, then Go to Examination -> Permission form
5. Students need to select their course
6. Check all details in the form
7. Add (upload) all Post Holding Certificates and Post Completion Certificates (except the last Post Completion, which you shall submit at the time of submission of the CPS examination form)
8. Click on Submit
The students need to take the print off the form and put their signature on it. Then, take the sign of their institute head along with the institution stamp and upload a scanned copy of the same in PDF format.
Checklist of documents (hard copies) to be submitted to CPS office for verification before 05-January-2024 up to 4:00 pm –
1. FCPS students: 6 Post Holding Certificate + 5 Post Completion Certificate (All Originals)
2. Diploma students: 4 Post Holding Certificate + 3 Post Completion Certificate (All Originals)
3. Permission form - Signed and Stamped by Institution Head (Original)
4. State Medical Council Registration / State Medical Council Registration Renewal (xerox copy)
5. Lecture Card (xerox copy)
6. Marksheet of DPM-PART-I (for the students who have passed the DPM Part-I examination and want to submit the permission form for DPM Part-II) [xerox copy of original marksheet]. Computer-generated marksheets will not be accepted.
7. Marksheet of DMRE-PART-I (for the students who have passed the DMRE Part-I examination and want to submit the permission form for DMRE Part-II) [xerox copy of original marksheet]. Computer-generated marksheets will not be accepted.
8. Aadhar Card (xerox copy)
The permission form shall be processed by the CPS office after receiving hard copies of the listed documents. The fee payment option shall be given to the students only after receiving the hard copies.
The students need to keep checking their CPS login after submitting the permission form to get an update on the payment option. When the students get the “Pay fees” option, they may make online payment of permission form fees. They shall get the permission form fee receipt after the payment of fees.
Guidelines for online payment of fees –
The CPS website links to a secure payment gateway website to collect fee payments. Upon completion of the payment transaction, the students will be redirected to the CPS website with a fee receipt. Please save and print this receipt for reference.
In case the transaction is successful and the amount is deducted from the bank account, but the students are not redirected to the CPS website with the fee receipt, they need to wait for 1 hour. In this case, please do not reinitiate payment or refresh the page to avoid duplicate payment.

Fee payment via online mode is preferable. If the students cannot pay permission form fees online, they may send a Demand Draft (DD) of fees to the CPS office, Mumbai, before 05-January-2024 (4:00 pm).
CPS is not responsible in any condition if the DD does not reach to CPS office in Mumbai within the stipulated period of time. Permission forms submitted without payment of fees shall not be considered for further processing. DD of requisite fees should be drawn in the favour of the “College of Physicians and Surgeons of Mumbai” payable at Mumbai. Receipt of fees by DD shall be subject to clearance of DD.
Fees details -
1. Permission form fees for Diploma - Rs. 3,200/- (In words – Three Thousand Two Hundred Only)
2. Permission form fees for FCPS - Rs. 13,200/- (In words – Thirteen Thousand Two Hundred Only)
The fees mentioned above are non-refundable.
After making payment, the candidates must save the Permission fee receipt for future reference.
