Low-Volume Cancer Center Matches Global Benchmarks in Curative Colorectal Surgery: Study

A recent retrospective cohort study published in the Indian Journal of Surgical Oncology in February 2025 demonstrates that resource-limited, low-volume centers can achieve elite surgical precision, delivering a 99.5% margin-negative (R0) resection rate and a 90.8% optimal nodal yield (≥12 nodes), matching global benchmarks for curative colorectal care.
Colorectal cancer (CRC) is India’s fourth most common cancer, yet five-year survival remains low due to lifestyle shifts and inconsistent outcome reporting. To address this clinical gap, Vishnu S. Menon and colleagues at Homi Bhabha Cancer Hospital & Research Centre audited their Tier-2, low-volume unit to evaluate if regional centers can meet international surgical benchmarks.
Therefore, the retrospective cohort study evaluated 207 curative resections for colorectal malignancies—including adenocarcinoma, melanoma, and neuroendocrine tumors—while excluding recurrent or benign cases. Utilizing electronic medical records and prospective databases, researchers assessed 30-day morbidity and mortality as primary endpoints, benchmarking outcomes against international high-volume standards.
Key Clinical Findings of the Study Includes:
Oncological Precision: The study achieved an impressive 99.5% rate of margin-negative resections and secured an optimal nodal yield of 12 or more in 90.8% of cases, demonstrating high surgical quality.
Morbidity Profile: Major perioperative morbidity, classified as Clavien-Dindo Grade 3a or above, occurred in 13% of the cohort, while the 30-day perioperative mortality rate was kept to a low 2.9%.
Recovery Metrics: Patients experienced a median hospital stay of 7 days, with the study reporting a low 3.4% readmission rate and a 7.2% re-exploration rate, which was often related to stoma complications.
Surgical Approach: While minimally invasive surgical (MIS) techniques faced a 16.7% conversion rate, they successfully reduced blood loss and shortened hospital stays compared to traditional open procedures.
Complex Interventions: Despite the center's low-volume status, extended resections were successfully performed in 15.9% of patients with outcomes that the study found comparable to those of established high-volume institutions.
The results suggest that even in the absence of a dedicated colorectal unit, a multidisciplinary approach can yield safe outcomes, with 90.8% of patients receiving an adequate lymph node harvest and nearly all achieving clear margins.
These findings imply that establishing standardized surgical audits and interdisciplinary tumor boards in evolving centers can effectively bridge the quality gap in regional cancer care.
While the retrospective nature and lack of long-term survival data are noted limitations, the findings highlight a vital opportunity for developing India-centric registries to further refine surgical training and resource allocation across emerging healthcare hubs.
