Systematic Umbrella Review Supports Robotic-Assisted Total Hip Arthroplasty for High-Risk Patients

India: Robotic-assisted total hip arthroplasty (RTHA) provides more accurate implant positioning, reduces leg-length discrepancy, and lowers short-term complications compared with conventional total hip arthroplasty (CTHA), a new systematic umbrella review has revealed. Although robotic surgery was associated with longer operating times, the findings suggest that its improved surgical precision may translate into modest but measurable clinical benefits, particularly in patients at higher risk of implant misalignment.
The findings, published in the Journal of Robotic Surgery, are from an umbrella review led by Dr. (Prof.) Raju Vaishya of the Department of Orthopaedics, Indraprastha Apollo Hospitals, New Delhi, and colleagues. The researchers synthesized evidence from previously published systematic reviews and meta-analyses to evaluate whether robotic-assisted hip replacement offers advantages over the conventional approach.
The review was conducted in accordance with Cochrane methodology. Researchers searched PubMed, Scopus, and the Cochrane Library, ultimately including 11 systematic reviews after PRISMA-guided screening. The analysis assessed outcomes including implant alignment, complications, revision surgery, operative time, leg-length discrepancy (LLD), Forgotten Joint Scores, and radiographic parameters such as cup inclination and anteversion. The methodological quality of the included reviews was evaluated using the AMSTAR-2 tool.
Key findings from the umbrella review include:
- Robotic-assisted THA significantly improved implant placement within the Lewinnek and Callanan safe zones compared with conventional surgery.
- The robotic approach reduced overall postoperative complications.
- Patients undergoing robotic-assisted surgery had less leg-length discrepancy, indicating more accurate restoration of limb length.
- Robotic-assisted THA was associated with better early patient-reported outcomes, reflected by improved Forgotten Joint Scores.
- The procedure required approximately 16 minutes longer operative time than conventional THA.
- No significant differences were observed between robotic and conventional surgery in revision rates, cup inclination or anteversion, or vertical center of rotation measurements.
The authors also reported that statistical analyses supported the robustness of the main findings, although publication bias varied across some outcomes.
Commenting on the findings, Dr. (Prof.) Raju Vaishya told Medical Dialogues, "Robotic-assisted total hip arthroplasty (RTHA) improves implant positioning accuracy, reduces leg-length discrepancy, and lowers short-term complications compared with conventional THA, although it requires longer operative time."
Dr. (Prof.) Filippo Migliorini added, "This umbrella review supports selective use of robotic-assisted THA, particularly in patients at high risk of implant misalignment, while highlighting the need for long-term randomized trials and cost-effectiveness data."
Echoing the clinical significance of the findings, Mr. (Prof.) Vikas Khanduja said, "These findings reinforce the paper's relevance by providing high-level evidence that robotic-assisted THA enhances surgical precision and early clinical outcomes, supporting its role in optimizing hip arthroplasty in appropriate patients."
The researchers noted that robotic-assisted THA requires longer operative time and that current evidence is largely limited to short-term outcomes. They called for long-term randomized trials to evaluate implant longevity, revision rates, and cost-effectiveness.
Overall, the review suggests that robotic-assisted THA improves surgical precision and early clinical outcomes compared with conventional hip replacement. The authors recommend its selective use, particularly in patients at high risk of implant malposition.
