USA: Intravenous arginine did not significantly reduce the time to resolve acute pain episodes in children and young adults with sickle cell disease (SCD) compared with placebo, a randomized clinical trial published in JAMA has shown. The study, which was stopped early because the treatment was unlikely to demonstrate the expected benefit, found no significant differences in opioid requirements, pain scores or patient-reported outcomes between the two groups.

The study was led by Claudia R. Morris from the Department of Pediatrics, Division of Pediatric Emergency Medicine, Emory University School of Medicine, Atlanta, Georgia, and colleagues.
Acute pain episodes are a major cause of emergency department visits and hospital admissions among patients with SCD. However, approved treatment options specifically targeting these episodes remain limited in the United States. Previous studies suggested that arginine supplementation could help manage acute pain crises, as patients may develop an arginine deficiency during these episodes. Earlier single-centre trials also reported potential benefits, including reduced opioid consumption and shorter hospital stays.
To investigate these effects, researchers conducted a phase 3, double-blind, randomized clinical trial across 10 children's hospitals in the United States between June 2021 and June 2024. The study enrolled patients aged 3 to 21 years who presented to emergency departments with SCD-related acute pain requiring injectable opioids.
Of the 274 participants randomized, 271 received the study treatment. Participants were assigned to intravenous arginine (129 patients) or a saline placebo (142 patients). The arginine group received an initial dose of 200 mg/kg, followed by 100 mg/kg every eight hours until discharge.
The primary outcome was the time to crisis resolution, measured from administration of the first study dose to the final intravenous opioid dose. Researchers also assessed total opioid consumption, pain scores, patient-reported outcomes and safety events.
The results showed the following findings:
  • No significant reduction in pain crisis duration: Median time to crisis resolution was 60.8 hours with arginine and 65.8 hours with placebo.
  • Similar opioid requirements: Total parenteral opioid consumption did not differ significantly between the groups.
  • No improvement in pain scores: Researchers found no significant differences in pain assessments or patient-reported outcomes.
  • No new safety concerns: Safety events were comparable between the groups.
The trial was discontinued early for futility after the interim assessment indicated that arginine was unlikely to achieve the anticipated improvement in crisis resolution time.
The findings suggest that intravenous arginine does not provide a significant clinical advantage over placebo for treating acute SCD-related pain episodes in children and young adults. Despite encouraging results from earlier studies, this larger multicentre trial did not establish a benefit in shortening pain crises or reducing opioid use.
The researchers concluded that intravenous arginine did not shorten the duration of acute pain episodes in young patients with SCD, highlighting the need for further research into effective treatments for this debilitating complication.
