Tailored Frontal Sinus Surgery Drives Recovery in Nasal Polyposis, Suggests Study

A recent prospective study published in the Indian Journal of Otolaryngology and Head & Neck Surgery in September 2026 shows that tailoring endoscopic frontal sinus surgery drives profound clinical recovery in nasal polyposis. Personalizing surgical selection reduced six-month disease burden from a baseline Lund–Mackay Score of 13.9 ± 4.3 down to 4.4 ± 1.6.

Management of severe sinonasal disease with frontal sinus involvement remains challenging, creating an ongoing need to compare distinct surgical procedures to optimize patient selection. To address this comparative evidence gap, lead author Mohammad Mandegari and colleagues from the Department of Otolaryngology and Head & Neck Surgery evaluated clinical outcomes across four endoscopic frontal sinus surgery approaches in patients with nasal polyposis.

Therefore, the prospective study evaluated 62 patients with nasal polyposis across four Draf procedure cohorts (I, IIa, IIb, and III). Patient outcomes—including Lund–Mackay Score (LMS), SNOT-20, SF-36, ostium patency, and complications—were monitored preoperatively and at 1 and 6 months postoperatively.

Key Clinical Findings of the Study Includes:

  • Radiologic Decongestion: Investigators recorded significant reductions in mean LMS from 13.9 ± 4.3 preoperatively to 7.2 ± 2.0 at 1 month and 4.4 ± 1.6 at 6 months postoperatively (P = 0.001).
  • Symptomatic Relief: Researchers observed marked improvements in mean SNOT-20 scores, dropping from 53.8 ± 14.4 preoperatively to 32.8 ± 8.8 at 1 month and 19.6 ± 5.5 at 6 months (P = 0.001).
  • Quality-of-Life Enhancement: Analysts reported significant increases in mean SF-36 scores, rising from 53.3 ± 12.8 preoperatively to 62.6 ± 14.2 at 1 month and 69.5 ± 14.7 at 6 months (P = 0.001).
  • Demographic Stratification: Authors identified a mean patient age of 47.0 ± 12.0 years (P = 0.024), with Draf III recipients representing the oldest subset.
  • Universal Procedural Efficacy: Evaluators confirmed that all four endoscopic frontal sinus surgical approaches achieved significant radiological, sinonasal, and functional benefits across patients.

The results suggest that tailored endoscopic frontal sinus surgery consistently drives marked therapeutic gains, as evidenced by 6-month postoperative reductions in LMS to 4.4 ± 1.6 and SNOT-20 scores to 19.6 ± 5.5. Consequently, surgical selection should be customized based on disease severity, surgical history, and comorbidities.

Thus, the study concludes clinicians may utilize these outcome metrics to guide operative decision-making, matching surgical extent to patient profile and disease burden to optimize care.

While the observational cohort reflects outcomes across diverse techniques, further prospective long-term trials in expanded populations would help refine personalized surgical algorithms for complex frontal sinus disease.
