USA: The Infectious Diseases Society of America (IDSA) has updated its clinical practice guidelines for the treatment and management of complicated urinary tract infections (cUTIs) in adults, replacing the previous 2011 iteration. Redefining cUTI to encompass symptoms extending beyond the bladder—such as pyelonephritis, bacteremia, or systemic fever—the updated recommendations emphasize tailored antibiotic stewardship, shorter treatment durations, and early de-escalation strategies. By aligning empirical drug selection with local resistance patterns and clinical disease severity, the framework aims to optimize clinical outcomes while minimizing broad-spectrum exposure and drug resistance.

The recommendations are discussed in a JAMA Clinical Guidelines Synopsis published in JAMA by Priya V. Nagarajan of NorthShore University HealthSystem–Evanston Hospital, Illinois, and colleagues. The updated guidance addresses the growing challenge of antimicrobial resistance and provides a structured approach to selecting antibiotics, switching from intravenous to oral treatment, and determining treatment duration.
Urinary tract infections remain a major global health burden. An estimated 405 million cases occurred worldwide in 2021, resulting in approximately 236,790 deaths. In the United States, UTIs accounted for 2.8 million hospitalizations in 2018, including 626,520 admissions for complicated infections. UTIs are also responsible for about half of gram-negative bloodstream infections in the country.
The IDSA panel included specialists in infectious diseases, hospital medicine, primary care, emergency medicine, and urology. Recommendations were developed using systematic reviews and the GRADE framework, with the strength of recommendations classified as conditional and the certainty of evidence ranging from moderate to very low.
The updated guidelines define uncomplicated UTI as an infection confined to the bladder, while cUTI includes infections extending beyond it, such as pyelonephritis and febrile or bacteremic UTI. Prostatitis is not covered by these recommendations.
Key recommendations include:
- Individualized empirical antibiotic selection: Clinicians should assess illness severity, resistance risk, patient-specific factors such as allergies and drug interactions, and local resistance data. For patients with sepsis, suggested options include third- or fourth-generation cephalosporins, carbapenems, piperacillin-tazobactam or fluoroquinolones. Previous urine culture results should guide treatment where available.
- Use local resistance patterns: For patients with sepsis, empirical treatment should reflect recent local antibiograms. The selected antibiotic should cover more than 80% of likely pathogens in sepsis without shock and approximately 90% in septic shock.
- Early switch to oral antibiotics: Clinically improving patients who can take an effective oral antibiotic may transition from intravenous treatment. Fluoroquinolones or trimethoprim-sulfamethoxazole are preferred oral options, with alternatives including amoxicillin-clavulanate and oral cephalosporins when appropriate.
- Shorter treatment courses: Patients with cUTI without bacteremia who improve on effective therapy may receive 5–7 days of fluoroquinolones or 7 days of non-fluoroquinolone antibiotics. For gram-negative bacteremic cUTI with clinical improvement, a 7-day course is preferred.
- Review treatment response: Patients who fail to improve promptly should be evaluated for complications such as urinary obstruction or abscess that may require additional intervention.
Evidence supporting shorter courses found similar microbiological cure rates with treatment lasting seven days or less compared with longer regimens. However, several studies excluded patients with severe sepsis, immunocompromise, urinary catheters and structural urinary abnormalities, limiting the applicability of these findings to some high-risk groups.
The guidelines also highlight antibiotic-specific safety concerns. Fluoroquinolones can cause tendon injury, QT prolongation and neurological adverse effects, while trimethoprim-sulfamethoxazole may cause severe skin reactions, blood-cell abnormalities and elevated potassium levels. Nitrofurantoin is not recommended for cUTI because it does not achieve adequate concentrations in blood and deep tissues.
The authors noted that further trials are needed to establish optimal treatment durations and intravenous-to-oral switching strategies in patients with severe infections, abscesses, indwelling catheters or anatomical abnormalities. Overall, the updated guidance promotes individualized antibiotic selection, timely treatment adjustment and shorter courses where supported by clinical response, aiming to improve outcomes while limiting unnecessary antibiotic exposure and resistance.
