Enhancing Musculoskeletal Cancer Recovery with Specialized Recovery Protocols, Suggests Study
A recent prospective observational cohort study in the Indian Journal of Surgical Oncology in May 2026 reveals that achieving a 77.5% adherence threshold to specialized recovery protocols slashes median hospital stays to 7 days and significantly reduces postoperative complication severity for musculoskeletal oncology patients.
Enhanced Recovery After Surgery (ERAS) is a multimodal, evidence-based pathway designed to mitigate surgical stress and preserve physiology for faster recovery. While effective in elective orthopedics, its application in musculoskeletal oncology remains under-investigated due to the high risks of major blood loss and systemic morbidity. To bridge this clinical gap, Dr. Sohan Lal Solanki and colleagues at the Tata Memorial Centre evaluated the impact of protocol compliance on postoperative morbidity.
Therefore, the prospective observational cohort study followed 300 adult oncology patients with bone or soft tissue tumors between December 2021 and May 2023 at a tertiary center to evaluate adherence to 20 specific perioperative interventions. Excluding emergency cases and inoperable tumors, the investigation prioritized primary compliance percentages and the severity of postoperative complications as defined by the Clavien-Dindo (CD) classification system.
Key Clinical Findings of the Study Includes:
Shorter Hospitalization: Study data revealed that high compliance levels shortened the median hospital stay to 7 days, compared to the 10 days observed in the low-compliance group.
Decreased Complication Severity: Study findings demonstrated a significant negative correlation between protocol adherence and postoperative morbidity, with 81.7% of patients experiencing only minor complications.
Lower Readmission Rates: Study results indicated that higher adherence to the recovery pathway significantly reduced the risk of 30-day hospital readmissions, particularly within the bone tumor cohort.
Superior Perioperative Adherence: Study observations achieved 100% compliance in critical areas such as preoperative lifestyle counseling, skin preparation, and the maintenance of intraoperative normothermia.
Early Functional Mobilization: Study records documented that 70.3% of patients successfully initiated mobilization and limb physiotherapy within the first 24 hours after their surgical procedure.
The results suggest that an overall compliance rate of approximately 84.5% is achievable for bone and soft tissue cancer surgeries and serves as a strong predictor of superior postoperative outcomes. Specifically, maintaining adherence above the identified 77.5% threshold is essential for clinicians aiming to minimize major adverse events and expedite patient discharge.
The findings suggest that adopting these standardized recovery protocols can enhance the quality of perioperative care for patients facing complex musculoskeletal cancer resections.
While the single-center study demonstrates clear benefits, the use of self-reported data and the diverse complexity of tumor types indicate a need for future multi-center research that stratifies patients by surgical difficulty to further validate these recovery trends.
