Infant-Onset Atopic Dermatitis Linked to Greater Disease Severity in Children: Study

A recent cross-sectional study published in the Indian Journal of Paediatric Dermatology in June 2026 reveals that while pediatric Atopic Dermatitis (AD) is typically mild and chronic, developing the condition during infancy acts as a major red flag for a much more severe disease course. The crucial finding provides clinicians with an actionable, early warning sign to proactively monitor and treat highly vulnerable young patients.
While pediatric AD is known to have a lower prevalence of 2.4% to 6% in India compared to developed nations due to distinct climatic and lifestyle factors, there remains a significant scarcity of clinico-epidemiological data from the Indian subcontinent. Addressing this critical gap, researchers Meghana Reddy, Namrata Chhabra, and Anil Kumar Goel from the All India Institute of Medical Sciences (AIIMS) in Raipur aimed to evaluate the epidemiology, clinical patterns, risk factors, and disease severity of this chronically relapsing skin condition in Central India.
Therefore, the researchers conducted a cross-sectional study over one and a half years at a tertiary care center in Chhattisgarh, evaluating 200 pediatric patients aged 0 to 15 years diagnosed using Hanifin and Rajka’s criteria, aiming to assess disease severity via the SCORAD index as a primary endpoint alongside identifying associated demographic risk factors. Statistical analyses were performed using SPSS software to interpret the clinical presentations and evaluate severity variables among the study population.
Key Clinical Findings of the Study Includes:
Predominant Severity Profile: The researchers found that the vast majority of cases (69.5%) presented with mild severity, while 28% and 2.5% of patients exhibited moderate and severe presentations, respectively.
Early Age of Onset: Investigations revealed that 61% of children developed the condition before the age of five, and 34.5% experienced disease onset during infancy before one year of age.
Disease Chronicity: The team noted a chronic disease course in a significant portion of the cohort, with 64% of the evaluated children suffering from the condition for more than a year.
Impact of Infantile Onset: The analysis demonstrated that an infantile onset of the condition serves as a significant clinical predictor, being substantially more associated with a moderate-to-severe disease grade (47.5%) compared to those with a later age of onset.
Environmental Risk Factors: The authors observed that although urban residence, an absence of exclusive breastfeeding, and excessive hygiene practices like frequent sanitizer use were more frequently noted in the moderate-to-severe patient cohort, these comparative differences did not reach statistical significance.
The results suggest that pediatric AD in Central India is characterized by an early onset, with 61% presenting before 5 years of age, alongside a chronic but predominantly mild clinical course in nearly 70% of the cases. Furthermore, the study identifies an onset during infancy as a definitive risk factor predisposing children to moderate-to-severe disease manifestations.
Thus, the study concludes healthcare providers to remain particularly vigilant when evaluating infants with early skin barrier dysregulation, as closely monitoring these younger patients could help in mitigating the potential progression to more severe clinical phenotypes over time.
While the current hospital-based findings offer valuable regional insights, observing broader community-based populations across different age groups in future studies could help in further substantiating these identified risk factors and severity profiles without diminishing the immediate clinical relevance of the present data.
