Herbal Formulations Match Synthetics in Plaque Control, Suggests Review

A recent narrative review published in the Journal of Dental Sciences and Oral Rehabilitation in July 2026 reveals that biocompatible herbal formulations reduce plaque and gingival bleeding as effectively as synthetic standards. These botanical alternatives offer clinicians a powerful therapeutic tool without the risks of tooth staining or mucosal irritation.

While global reports from the World Health Organization (WHO) show untreated biofilm-mediated oral infections affect billions worldwide, prolonged synthetic chemical use triggers tooth staining, dysbiosis, and antimicrobial resistance. To address this clinical gap, Dr. Meghna Bhargava and colleagues from the Department of Pediatric and Preventive Dentistry at the Institute of Dental Sciences in Bareilly, India, reviewed the efficacy, safety, and future of diverse plant-derived dental formulations.

Therefore, the narrative review analyzed clinical and laboratory studies of botanical dentifrices, gels, mouthwashes, and intracanal therapies against conventional synthetics, excluding non-standardized extracts. Key evaluations focused on plaque and gingival index scores, microbial reduction, root dentin microhardness, and cell safety.

Key Clinical Findings of the Review Includes:

  • PI and GI Reduction: The review indicates that herbal toothpastes containing neem, triphala, or green tea significantly decrease PI and GI scores compared to baseline, achieving clinical efficacy equivalent to conventional standards.

  • Prolonged Gel Activity: Evidence demonstrates that topical gels formulated with neem, aloe vera, or tulsi provide extended tissue contact, yielding reductions in gingival inflammation comparable to the chlorhexidine gold standard.

  • Mouthrinse Biofilm Control: The analysis confirms that mouthwashes with triphala, propolis, or green tea achieve short-term plaque and gingival bleeding reductions equivalent to synthetic rinses.

  • Endodontic Pathogen Elimination: Trials suggest that combined nutmeg and neem gels or miswak extracts effectively inhibit resistant Enterococcus faecalis biofilms, matching or exceeding calcium hydroxide performance.

  • Root Dentin Preservation: The evaluation reveals that babul extract exhibits significantly fewer adverse effects on dentin microhardness while displaying superior cytocompatibility compared to conventional calcium hydroxide.

The results suggest that herbal dental formulations exhibit potent antimicrobial, anti-inflammatory, and antioxidant properties, matching the clinical performance of chlorhexidine and fluoride in short-term assessments. These findings establish plant-derived agents as promising, biocompatible alternatives for daily oral care, although further research is required to translate these preliminary results into standard protocols.

Thus, the review concludes clinicians may consider incorporating these botanical formulations as biocompatible, well-tolerated adjuncts to conventional mechanical plaque control and endodontic therapies to help mitigate synthetic-related side effects such as tooth discoloration and mucosal irritation.

While the present review is limited by the inherent variability in phytochemical compositions and a current shortage of long-term clinical data, future research leveraging nanotechnology, advanced phytochemistry, and large-scale, multicenter randomized controlled trials (RCTs) represents an attractive avenue to standardize dosing and establish robust regulatory safety guidelines.
