Rare Case Report Reveals Aplasia Cutis Congenita Presenting as Missing Skin on a Newborn's Foot

A recent case report highlights an extraordinary instance of Aplasia Cutis Congenita (ACC) where a newborn was completely missing skin on their foot rather than the typically affected scalp. This surprising presentation serves as a crucial reminder for healthcare providers to remain alert for atypical manifestations of rare congenital anomalies.
The findings of the case report are published in the Indian Journal of Neonatal Medicine and Research in March 2026 by Dr. Priyanka and colleagues from the Department of Paediatrics at the Post Graduate Institute of Medical Sciences in Rohtak, Haryana, India.
Recognizing this anomaly holds immense clinical significance because it underscores the necessity of identifying atypical congenital dermatological defects rapidly to prevent life-threatening complications like severe infections, bleeding, and thrombosis. Accurate identification allows physicians to rapidly exclude underlying syndromic associations or systemic malformations, ultimately preventing unnecessary invasive surgical interventions in favor of highly effective, less traumatic care strategies.
A full-term male newborn presented on his first day of life exhibiting a sharply demarcated absence of epidermal tissue covering two-thirds of his left foot; subsequent investigations, including lower limb Doppler ultrasonography and maternal serological panels, yielded completely normal results. Following a definitive clinical diagnosis of isolated lower extremity ACC, providers successfully initiated conservative treatment utilizing gentle wound debridement alongside topical fusidic acid, facilitating robust spontaneous epithelialization and complete, complication-free healing within weeks.
Although the precise pathogenic mechanisms driving this condition remain unresolved, prevailing evidence suggests the anomaly stems from either a primary failure of embryonic skin development or a secondary in utero degeneration of previously formed cutaneous tissue. Researchers attribute these disruptions to a complex interplay of potential etiologies, including specific genetic mutations involving the ribosomal GTPase BMS1 alteration, intrauterine vascular accidents, fetus papyraceus, maternal infections, and early teratogenic exposures.
The specific presentation fundamentally enriches pediatric dermatological paradigms by unequivocally demonstrating the extraordinary, rapid regenerative capabilities inherent to newborn skin when devoid of deeper structural malformations. These findings offer vital reassurance to frontline healthcare practitioners, strongly advocating that conservative, non-surgical wound care should represent the primary standard of care for similar localized defects, thereby optimizing patient outcomes and minimizing the burdens of surgical grafting.
