MUHS issues notices on registration, admission of students who have entered 1st year medical, dental, MSc Nursing degree courses

Mumbai: Through recent notices, Maharashtra University of Health Sciences (MUHS) has informed regarding the registration and admission of students who have entered the first-year medical degree course, postgraduate degree MSc (Nursing) course, dental degree course in the college affiliated to the university in the academic year 2020-21.

The MUHS has laid down the instructions and list of documents while submitting the application form along with other important details as attached below:

For students in Medical courses

Instructions for filing the application form

Annexure-II

1. At Sr. No. 1 of the Application Form Write name in block letter as per qualifying HSC Certificate (attach documentary proof).
2. Submit separately a set of original documents & a set of photocopies arranged serially as mentioned below (from Sr. No. 01 to 11)
3. Set of legible photocopies of original documents must be duly attested by the Dean / Principal / Director & attached with the application form.
4. Attach original documents at 11, 12 and 13 in below list, to the application form.

List of documents to be submitted along with the application form

1. a) Nationality Certificate issued by District Magistrate, Additional District Magistrate, Chief Metropolitan Magistrate or
b) Photo copy of Valid Passport duly attested by Dean or Principal or Director or
c) Birth Certificate having endorsed with nationality as mention "Indian" on it.
2. Domicile Certificate
3. S.S.C. Passing Certificate (10th )NEET UG-2019 Mark sheet.
4. H.S.C.Mark Statement (12th )
5. NEET UG-2019 Marksheet
6. Selection Letter
7. Medical Fitness Certificate. (as per the format given in the Brochure of State CET Cell, Mumbai i.e. ( Annexure – "H" )
8. Caste Certificate (If applicable)
9. Caste Validity Certificate (If applicable) (It is mandatory to the Student belonging to reserved category to submit Caste Certificate duly supported by Caste Validity Certificate. Failing which proposal will not be accepted)
10. Non-Creamy Layer Certificate valid up to 31/03/2020 for (VJ)DT-A, NT-B, NT-C, NT-D, OBC, SBC
11. College Leaving Certificate (LC / TC)
12. Migration Certificate issued by the respective University. (In original form) (If Applicable)
13. Self-Educational Gap Affidavit by student certified by Executive Magistrate / Notary. (If applicable)
14. Physically Handicapped Certificate ( as per the format given in the Brochure of State CET Cell, Mumbai i.e. Annexure – "D") (If applicable)
15. M.K.B. Certificate (Maharashtra Karnataka Disputed Border Area)
16 Person belong to Hilly Area (Parents Domicile)
17 DEF1, DEF2, DEF3

MSc Nursing
1. The candidate should be a Registered Nurse and Registered midwife or equivalent with any State Nursing Registration Council.
2. The minimum education requirements shall be the passing of: B.Sc. Nursing / B.Sc. Hons. Nursing / Post Basic B.Sc. Nursing with minimum of 55 % aggregate marks.
3. The candidate should have undergone in B.Sc. Nursing / B.Sc. Hons. Nursing / Post Basic B.Sc. Nursing in an institution which is recognized by Indian Nursing Council.
4. Minimum one year of work experience after Basic B.Sc. Nursing.
5. Minimum one year of work experience prior or after Post Basic B.Sc. Nursing.
6. Candidate shall be medically fit.
7. 5% relaxation of marks for SC/ST candidates may be given.

4 Valid Registration certificate from the Central/State Council (Provisional Registration Certificate will not be considered)
5 Caste Certificate (If applicable)
6 Caste Validity Certificate (If applicable) (It is mandatory to the candidate belonging to reserved category to submit Caste Certificate duly supported by Caste Validity Certificate & Non-Creamy Layer, failing which proposal will not be accepted)
7 Valid Non-Creamy Layer Certificate for the Categories DT/VJ, NT, OBC, SBC
8 College Leaving Certificate (LC/TC)/Continuation letter by College Authority. (Annexure-C)
9 Attempt Certificate duly signed by Head of the Institute (If applicable)
10 Copy of Gazette, Marriage Certificate & Affidavit in case of change in name (If applicable)
11 Final Year Mark sheet of qualifying examination
12 Experience Certificate duly signed by the Head of the Institute
13 Migration Certificate issued by the respective University (Applicable to Non-MUHS students only)
14 Affidavit Gap Certificate (made by the student duly certified by Executive Magistrate) (If applicable)
15 Medical Fitness Certificate duly quoted with Registration number
16 Physically Handicapped Certificate (If applicable)
17 Documentation for In-Service personnel (If applicable)
a) Deputation Certificate
b) No objection Certificate
c) University approval Certificate for teacher in the concerned subject
